Will provide community support to those with severe emotional and mental issues as well as substance abuse issues. This is a full time, Monday to Friday job, working one evening until 8 AM a week. This position entails visiting clients in the home, follow ups after client is hospitalized and working with clients to develop and implement a treatment plan for recovery.
GENERAL SUMMARY AND SCOPE
Responsible for assessing, coordinating, and monitoring the various health and human services needs of behavioral health patients. Works toward achieving OhioMHAS Mental Health & Recovery Services Boards, and any special requirements of grants. Outcomes include: increasing community tenure; encouraging vocational involvement; developing a social support network; examining housing and financial needs; coordinating care for physical health needs; engaging in treatment and self-help services; and assistance in interfacing with health and social service systems. Responsible for developing an appropriate service plan with client input, that is recovery focused. Responsible for complying with all requirements of the continuous performance improvement program.
KNOWLEDGE, SKILLS AND ABILITIES
Experience in a behavioral health or human service field required. Behavioral Health related degree preferred.
CPR and First Aid certification within 6 months of hire.
Computer skills sufficient to learn and utilize the electronic medical record are a mandatory requirement for all direct service staff and supervisory staff.
Punctual attendance at assigned work location is required.
Ability to be mobile and possesses a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
