Position Summary

This position is funded by a grant for a duration of time as defined by the grantor.

In collaboration with the Manager, will provides coordination activities for the developing and organizing of the day-to-day operations of the TennCare Presumptive Eligibility (TPE), Tennessee Breast and Cervical Cancer Screening (TBCSP) inclusive of the Women's Health Navigation Expansion Activities.

Pay Grade: 45

Salary Commensurate with Experience and Education

This position is eligible for a HIRING BONUS outlined below:

1.1st payment of $1,500 paid after completion of six (6) months of service.

2.2nd payment of $1,500 paid after completion of twelve (12) months of service.

3.3rd payment of $2,000 is paid after completion of eighteen (18) months of service.

Minimum Qualifications

100% -

1.Two (2) years of experience in health promotion or health education capacity; AND

2.Bachelor's degree from an accredited college or university in public administration, health administration, business administration, social work or closely related field; OR

3.An equivalent combination of related education, and/or experience.

4.Proof of education, training, and/or experience is required.

Duties and Responsibilities

1.Enters data for the TennCare Presumptive Enrollment services, including Prenatal, Breast and Cervical Cancer, and Women's Health Expansion Activities, and assists with covered children enrollment.

2.Provides case management and follow-up services for women with abnormalities related to breast or cervical cancer screening examinations or tests. Refers women to specialists, authorizes services as needed, and provides patient education and support. Maintains contact with women and specialists' offices throughout patients' diagnosis and treatment. Works with patients and providers as concerns arise, and obtains all reports of tests, procedures, and visits to specialists.

3.Provides referral, authorization of services, patient education, continuing service planning, and other appropriate interventions in cooperation with a network of specialists and primary care providers.

4.Assists with the mobilization activities and collaborates with community partners (i.e., screening sites, Shelby County Health Department- Family Planning Section, schools/universities, hospitals & clinics) to identify and address health challenges impacting customers within targeted populations.

5.Provides patient navigation by contacting providers, scheduling appointments, assessing for barriers, and follow-up for all eligible clients.

6.Assists with the provision of evidence-based client education and support with the expansion activities related to evidence-based public education and community outreach to disproportionately burdened populations, to the implementation of social marketing activities.

7.Coordinates TBCSP activities of participating local health departments (LHDs), FQHCs, or organizational clinics.

8.Assists with the training of participating sites/clinics and referral providers in program policies.

9.Assists with the monitoring of patient delivery and data collection activities.



  1. Performs evidence-based outreach and engagement activities with a special focus on disproportionately burdened populations; participates in pertinent coalition and workgroup meetings such as the Tennessee Cancer Coalition (TC2).

  2. Responsible for the daily entering of transmittal data in designated databases and with data collections for quality improvement activities.

  3. Serves as a Certified Application Counselor to help enroll clients. Responsible for obtaining certification upon hire.

  4. Completes assessment and referrals for clients needing diagnostic tests and services.

  5. Participates and attends mandatory State-mandated conferences and trainings.

  6. Serves as the liaison between the Central Office (Tennessee Department of Health-Nashville) and local sites/clinics for program management.

  7. Assists during public health emergencies and participates in agency training(s) as deemed appropriate by leadership.

  8. Performs other related duties as assigned or directed.

1.Knowledge of care coordination and navigational activities.

2.Knowledge of medical terms and procedures, specifically related to Breast and Cervical diagnoses and treatment plans.

3.Knowledge of public administrative functions, programs, policies, and procedures.

4.Ability to work as a team member with diverse populations.

5.Ability to resolve issues and interpret policies, maintain confidentiality, prioritize, and work independently while performing multiple tasks.

6.Ability to work in a fast-paced, professional, diverse, and self-directed environment with minimal supervision.

7.Knowledge of excellent oral, written, and interpersonal skills

8.Ability to demonstrate attention to detail.

9.Proficient knowledge of computer applications to include databases, and Microsoft Office Suite (Word, Excel, PowerPoint).

Physical Requirements

1.The physical activities of this position may include climbing, pushing, standing, walking, reaching, grasping, kneeling, ascending/descending stairs, stooping, and repetitive motion.

2.Requires lifting, navigating steps, stooping, bending, and lifting of items greater than 15 lbs.

Special Requirements

1.Must possess a valid Tennessee driver's license or secure one upon the date of employment.

2.Must have daily access to an operable automobile and meet county requirements for automobile insurance upon the date of employment.

3.Must be willing to carry a cell phone for contact.

4.Must be flexible and available to work some nights and/or weekends, community-based events.
