Job Description

ESSENTIAL FUNCTIONS

  • Assesses, plans and implements nursing strategies to improve the health status of individuals and populations.

  • Designs and implements health education campaigns and activities for disease prevention.

  • Completes goals and objectives for assigned grant programs.

  • Maintains cooperative working relationships among public and agency participants.

  • Ability to perform clinical duties, including but not limited to: physical assessments, immunization and medication administration, effective communication, and efficient use of electronic medical records.

  • Plans or participates in development of program proposals for funding, as assigned.

  • Not everyone performs all of these duties but duties assigned may include any of the following: HIV/STD Grant

  • Investigates positive laboratory tests for syphilis and HIV within a five county region; notifies patients of positive tests, interviews patients for partner notification, arranges and follows up on treatment, refers patients to community based services, maintains records, enters information into the Ohio Disease Reporting Systems (ODRS).
  • Participates in community STD educational activities.

  • Participates in projects designed to identify and test high-risk persons.

  • Performs tests for HIV, prepares reports and conducts communicable disease education programs and awareness campaigns Communicable Disease

  • Interrupts the spread of communicable disease by investigating suspected reportable diseases. Tasks include interview of patients, collection of specimens, interaction with physicians and infection control staff, maintenance of records, and preparation of reports.
  • Leads outbreak response efforts and coordinates with emergency preparedness staff when needed.

  • Coordinates interventions, surveillance, case management, data collection and reporting; serves as subject matter expert; provides technical guidance to community physicians and infection control practitioners.

  • Work effectively with people of diverse cultures, ages and economic backgrounds in a culturally competent and sensitive manner.

  • Establish and maintain effective working relationships with employees, supervisors, other agencies, and the public.

  • Possess integrity, sense of responsibility, independent judgment.

  • Communicates effectively and accurately both verbally and in writing.

  • Completes 100% of assigned grant objectives and deliverables timely (where applicable).

  • Completes 100% of assigned duties timely.

  • Attends all mandatory project meetings.

  • Maintains proficiency in 100% of required SCPH clinical competencies for medication administration and phlebotomy Accurately follows all applicable Standard Operating Procedures (SOPs) Responds to voicemails and emails within 24 hours.

  • Maintains all appropriate licensure and certification Adheres to all HIPAA requirements.

  • Completes annual goals and training as outlined in Employee Performance Evaluation.

  • Participates in Medicaid Administrative Claiming Activities such as referral, enrollment, monitoring, and coordination of services.

  • Works a set schedule to meet the needs of the program, division and agency.

  • Performs job duties under the regulations, policies and procedures set forth by Summit County Public Health and outside agencies such as the County Fiscal Officer, Ohio Department of Health, and state and federal government.

  • Performs any other duties as required for the efficient operation of Summit County Public Health Obliges, as set forth by the Summit County Public Health Emergency Operations Plan and within the scope of their employment, to be available for emergency service during routine and non-routine hours of operation including 24/7/365 availability.

  • May be required to have medical clearance for respiratory protection or other personal protective equipment and may be exposed to inclement weather, extreme temperatures, unpleasant smells or odors, dust, dirt, fumes, airborne particles, pesticides, toxins or caustic chemicals.

BASIC REQUIREMENTS

Baccalaureate Degree in Nursing required Active Registered Nurse license in State of Ohio A minimum of two years previous related work experience in the delivery of nursing services (preferred) Previous public health experience preferred Current valid Ohio driver's license, automobile availability and insurance coverage

WORKING ENVIRONMENT

The noise level in the work environment is usually quiet while in the office, other settings typically include higher noise levels

The employee may be required to lift items as heavy as 50 lbs
