Summary

Kids Plus is a multidisciplinary team of nurses and social workers, partnering with families and community providers to address the health and social needs of children and families experiencing homelessness. Our team specializes in medical case management, while working to address the many complexes needs and barriers that impact the health and well-being of children. This PHN is client-centered and meets families where they are to support each family's goals for well-being and stability. This PHN coordinates care, offers health education and screening, connects families to community resources and offers support to decrease the stress of living homeless for the entire family. This PHN also participates in community groups, coalitions, and advisory committees to strengthen the network of services available to family's experiencing homelessness.

Job Duties

Work Schedule: Initially, this 1.0 FTE, fully-benefited position will work Monday

  • Friday, 8 a.m.

  • 5 p.m. Following successful completion of the probationary period, opportunity to request an alternative work schedule is possible and based on operational needs. May work outside typical days/hours to meet operational needs. This position is overtime eligible.

Work Location: The position is based out of our Downtown Public Health Center 2124 4th Ave, Seattle, WA 98121 but will require travel across Seattle-King County.

Interviews for this opportunity will be held on July 17th and 18th.

Job Duties:

  • Kids Plus serves families and their children who are experiencing homelessness (living on the street, in their car, in a shelter, or in transitional housing) in their community. PHN must be comfortable working in these environments.

  • Provide client centered care to people from diverse backgrounds who frequently have a history of trauma.

  • Work as a member of a multidisciplinary team of social workers and PHNs.

  • Provide client-centered services to families through both outreach and/or case management models.

  • Provide medical case management: care coordination, linkage and referral, chronic disease management, developmental screening, and referrals.

  • Determine program eligibility and provide client-centered education, interventions, support, and referrals remotely (phone/video) and in office settings, client homes, and other community locations.

  • Document client information and services and manage appointment schedules in various electronic health records, including EPIC.

  • Participate in quality assurance activities to meet regulations, policies, and procedures.

  • May provide birth control and pregnancy options counseling.

Experience, Qualifications, Knowledge, Skills

Requirements:

  • Bachelor's degree in nursing science or a graduate level nursing degree of MSN, DNP or PhD from an accredited school of nursing.

  • Current Washington State license as a Registered Nurse or Multi-State Licensure.

  • Documentation of current BLS-CPR for Health Care Providers. CPR/AED HeartSaver courses do not meet this requirement.

  • Washington state driver's license or ability to travel to sites within the county.
