Position Description

The Anoka County Public Health & Environmental Services mission is to improve health through education, prevention, service, regulation, and advocacy in Anoka County. The department has multiple immediate openings for Public Health Nurse positions on the Family Home Visiting team. The Public Health Nurse helps reduce health risks at the population level through evidenced-based care and education. The Public Health Nurse is responsible for addressing the health disparities within marginalized communities by focusing on the environmental, physical, and social determinants of health.

Emergency Preparedness is a core Public Health responsibility. As a Human Services employee, your roles and responsibilities include: participating in Public Health Emergency Preparedness training and exercises, responding to notifications and activation drills during business hours and periodically outside of business hours, and responding to Public Health emergencies and department responsibilities at the time of need (during business hours, outside of business hours, on weekends and holidays) in order to protect the health and well-being of our residents.

This is a full-time, exempt, benefit-eligible position located at the Anoka County Government Center and will require travel to various sites throughout Anoka County. Duties may be performed during non-traditional hours.

Pay and Benefits

  • Salary: $70,012 to $84,011($33.66 to $40.39 per hour)

  • 2024 Anoka County Salary Schedule Grade 34. $70,012 to $98,009 ($33.66 to $47.12 per hour).

Work Location

  • This position will work at the Anoka County Government Center, located at 2100 3rd Ave, Anoka, MN 55303 with travel to various sites throughout Anoka County.

  • This position may be eligible for flexible work arrangements, including hybrid work, with some days working remotely and some days working in the office.

  • Expected work hours are Monday

  • Friday 8:00 a.m.

  • 4:30 p.m.

Duties and Responsibilities

These examples are not all inclusive and are intended to be illustrative of primary responsibilities of an Anoka County Public Health Nurse.

  • Provide assessment, counseling, health education and follow-up to family home visiting client caseload through evidence-based practice.

  • Provide family planning information and counseling to families and individuals as needed.

  • Assist young families with children by providing information and guidance on growth and development as well as social and emotional difficulties.

  • Develop nursing care plans by matching patient/client needs with available resources and provide or arrange the needed care.

  • Refer program participants to community agencies, health care providers and social services.

  • Assist in coordinating program services such as WIC, Public Health Nursing and Social Services staff.

  • Collaborate with community agencies such as schools, health care providers and others to meet the needs of the high-risk family home visiting clients.

  • Serve as a resource to the agency and community groups regarding maternal and child health concerns.

  • Assist in developing nursing protocol for management of high-risk pregnancy conditions and chronically ill/disabled children.

  • Represent the agency to the public, participates in educational presentations, and participates in ongoing program outreach.

  • Emergency Preparedness is a core public health responsibility. Responding to Public Health emergencies, which may occur outside of normal business hours, is a requirement of this position to protect the health and well-being of our residents.

  • Provide leadership, coaching, and/or mentoring as assigned.

  • Participate in immunization clinics.

  • Participate in the Public Health Emergency Preparedness (PHEP) planning and response activities for the Public Health and Environmental Services (PHES) Department and Anoka County, which may occur outside of normal business hours.

Qualifications and Requirements:

Minimum Knowledge, Skills, and Abilities Needed

  • Bachelor's degree in nursing.

  • Two years (2,080 annual hours) of job-related experience.

  • In lieu of the required education, an equivalent combination of education and experience may be substituted on a year-for-year basis.

  • Registered Nurse in the State of Minnesota.

  • Possess a Minnesota Public Health Nursing Certification.

  • Must have a valid driver's license and vehicle available for business use.

Preferred Knowledge, Skills, and Abilities Needed

  • Three years or more (2,080 annual hours) of job-related experience.

Physical Demands and Work Conditions

  • Standard office environment, travel to various county work sites both indoor and outdoor, may include emergency or disaster response sites in uncontrolled environments depending on assignment

  • Work activities may require contact with uncooperative, angry, and sometimes hostile clients depending on assignment

  • Travel between work sites may require driving a county vehicle and/or a personal vehicle
