The mission of the Boston Public Health Commission (BPHC) is to protect, preserve, and promote the health and well-being of all Boston residents, particularly the most vulnerable. The BPHC sets an expectation that all staff and leadership commit, individually and as part of the BPHC team, to hold ourselves accountable to establishing a culture of antiracism and advance racial equity and justice through each of our bureaus, programs, and offices. Program Description Healthy Baby Healthy Child provides home visiting to pregnant and parenting families with a child under the age of five who reside in Boston. All services are free, voluntary and confidential.

Services in health education, nutrition, breastfeeding counseling, injury prevention, SIDS grief counseling, among many others. Subprograms include Father Friendly Initiative, Welcome Family and others. Services offered in English, Spanish, French, Somali and others.

Duties Facilitates and advocates for client access to services that provide basic needs, education, housing, health care, legal and counseling assistance. Conducts home visits to assess the needs of families and provides services in collaboration with public health nurses and other service providers. Develops a Family Service Plan and intervention strategies for each client and family served by the program.

Delivers services according to established advocacy and case management protocols. Maintains accurate, legible and current documentation. Meets regularly with Supervisor for consultation and evaluation of Family Service Plans.

Provides client teaching and training on how to access community resources. Works collaboratively with clinical provider at the primary care sites through participation in case conferences. Implements outreach strategies to engage clients in community based services.

Performs other duties as required. This job description is based on the Public Health Advocate Series. May be required to perform any and all duties and responsibilities of a Public Health Advocate 1, as set forth in the BPHC Public Health Advocate Series.
