POSITION SUMMARY:

The Physical Therapist assesses the need for, develops, and delivers physical therapy programs to facilitate rehabilitation. The Physical Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of patients.

He/she implements individualized therapy programs designed to restore, reinforce, and enhance performance. In addition, the Physical Therapist instructs, educates, and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. He/she directs activities to promote and maintain health.

RESPONSIBILITIES/ACCOUNTABILITIES:
As necessary or applicable, supervision of Physical Therapy Assistants, temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.
Meets patient care hours (PCH) requirement for your rehab gym, or as directed by Program Manager/Area Director.
Attends and contributes to patient care, staffing conferences and other related meetings.
Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient.
Consults and collaborates with other members of the health care team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
Adapts program and/or plan of care according to the needs of individual patient
Promotes a safe environment and enforces the elimination of fire and safety hazards.
Orders supplies and equipment as necessary.
Cleans, maintains, identifies, and reports the need for repair of equipment as necessary.
Puts Patient Service first: ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights. Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the physical therapy treatment plan, and completes all related documentation and record keeping regarding these services.Services include but are not limited to:
Therapeutic exercise including aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, gait and locomotion training, neuromotor development training, relaxation, strength, power, and endurance training
Functional training
Manual therapy
Airway clearance techniques
Integumentary repair and protection
Use of electrotherapeutic, physical agent and mechanical modalities
Designs, fits and/or applies adaptive devices, and trains patients in the use of adaptive devices including orthotics and prosthetics.
Selects appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest.
Instructs patient in the skills and techniques required for participation in mobility activities, and evaluates patient progress, attitudes and behavior as related to rehabilitative potential.
Performs routine reexamination as needed/required to modify/progress plan of treatment.
Performs other related duties as required
EDUCATION:
Bachelor of Science degree from an APTA approved school of Physical Therapy
Experienced preferred (will consider new grad)

LICENSES/CERTIFICATES:
Fire Card, BCLS, State license; eligible for membership in the APTA.
Must adhere to code of ethics of the APTA

Responsibilities: Duties and Responsibilities: * Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load. * Responsible for the correct reporting of billing information to the Director of Physical Therapy in regard to all patients on personal case load. * Provide direct patient care as prescribed, including evaluation and treatment planning. * Assist and assess transfers, gait and balance as required by functional mobility of patient level of assistance from minimum to maximum. * Manual manipulation, including pushing and/or pulling of patient body parts and limbs. * Responsible for the reporting and interpretation of treatment results. * Participate in development and presentation of in-services for the facility as requested. * Supervise support personnel in immediate work areas as appropriate. * Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare and Therapy Management Corporation's policies and procedures. * Participate in meetings regarding patient care as appropriate. * Assist when appropriate with any community education and public relations activities of the facility. * Provide education to patient's family as well as staff on the specific treatment plan as well as therapy goals for the patient. * Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established. * Other duties as assigned. Qualifications: Qualifications: * A minimum of a Bachelors degree in Physical Therapy. * Current certification with the American Physical Therapy Association. * Licensed by the State in the designated geographic area in his/her respective therapy profession. * Demonstrates the ability to perform the following tasks without posing a threat to the health or safety of him/herself or others and without risk to self, co-workers, caregivers, or patient: lift, turn, move position and transport the patient into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. * Sedentary tasks related to job – approximately 10% * Physical tasks as listed – approximately 90% * Demonstrates the ability to perform a minimal assist transfer.
