Home healthcare PTs have the autonomy to make individualized patient-centered decisions to promote optimal clinical care.

Job Details:

Provides assessments to determine patients' rehabilitation needs and psychosocial status.

Assesses each patient's environment and recommends adaptive equipment and/or home modifications as needed.

Establishes an effective physical therapy treatment plan in partnership with the patient, their family/caregiver, the patient's physician and other members of the health care team; modifies the treatment plan as needed.

Provides physical therapy services in accordance with the treatment plan, agency standards, and regulatory guidelines.

Helps patient achieve independence at home through activities of daily living training, functional activity training, visual/perceptual training, and psychosocial interventions, and heat/cold application.

Provides patient and/or caregiver education regarding the injury/disease process, functional limitations and the purpose of treatment interventions. Trains patients and/or caregivers in aspects of care that will be managed independently after discharge.

Plans adequately for discharge including referrals to appropriate community resources.

Communicates regularly with the patient's physician, home health care team, and other key members of the care team.


EDUCATION: Bachelor's degree in Physical Therapy from an accredited school.

EXPERIENCE: 1 year clinical experience.

LICENSE/CERT/REG: License to practice Physical Therapy in the District of Columbia, Maryland or Virginia. CPR certification required.

SKILLS: Good clinical skills. Knowledge of home equipment, home care reimbursement, and regulations governing PTAs. Verbal and written communication skills. Basic computer skills preferred.

Primary Duties and Responsibilities

  • Communicates with supervisor as scheduled and needed regarding issues related to the delivery of rehab services and individual patient care.

  • Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance and other mandatory training. Complies with governmental and accreditation regulations.

  • Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

  • Maintains ongoing communication with the patient's physician, health care team, and other individuals ( case managers, referring facility personnel) as needed regarding aspects of care and patient's


  • Performs other duties as assigned.

  • Planning adequately for discharge including referrals to appropriate community resources (e.g. outpatient therapy, day care, driver's education) able to meet the patient needs after discharge. Demonstrates understanding of the resources available to patients based upon place of residence, in addition to the patient's financial resources.

  • Provides an assessment, in conjunction with the health care team, of the patient's environment and recommending adaptive equipment and/or home modifications that will maximize the patient's functional abilities and safety. Assisting in the attainment of those items that are reimbursable as orthotics/prosthetics or durable medical equipment.- Requires knowledge of equipment and home modification options including cost and availability, equipment coverage guidelines, and resources to obtain such devices/ adaptations.

  • Provides initial and periodic assessments to determine the patient's habilitation/rehabilitation needs.

  • Establishes an effective physical therapy treatment plan in conjunction with the patient, their family/caregiver, the patient's physician and other members of the health care team, and modifying the treatment plan as needed. Evaluation includes interview, observation, palpation, specialized assessments including but not limited to goniometry, muscle strength testing, and sensory testing. Such assessments require the use of

tools (e.g. goniometer, sphygmomanometer, stethoscope) and the possession of sight and hearing senses or the use of prosthetics that will enable these senses to function adequately.

  • Provides medical record entries according to agency standards, professional guidelines, and regulatory requirements for the reimbursement of physical therapy services. Ninety-five percent accuracy on technical

audit. Ninety-five percent accuracy on technical audit.

  • Provides patient and/or caregiver with written educational material regarding the injury/disease process, functional limitations, and the purpose of treatment interventions, in addition to, training in aspects of care the

patient/caregiver will need to manage after discharge. Demonstrates ability to provide verbal and written

education in English at an educational level understandable to the patient/caregiver.

  • Provides physical therapy services in accordance with the treatment plan, agency standards, and other professional and regulatory guidelines. Performs procedures and modalities, including but not limited to, therapeutic exercise, functional mobility training, gait training, heat/cold application, and ultrasound.

  • Demonstrates understanding of the guidelines governing the reimbursement of services rendered by a physical therapist working in the home care environment in the State of Maryland, Virginia, or the District of


  • Supervises physical therapist assistants in their provision of patient care services and home health aides in aspects of patient care and related activities. Demonstrates an understanding of laws and regulations

governing physical therapist assistants.
