PMHNP Needed for Norwell Outpatient and/or PHP Program (Adolescents & Adults)

Salaried Paid Position

Arbour Counseling Services in Norwell is now seeking a PMHNP to provide outpatient, mental health and substance abuse medication management to children and adults.

We are looking for enthusiastic, motivated individuals to join our team!

Applicants must have experience in assessment and treatment of dual diagnosis individuals who need emotional, psychological, and behavioral support. Applicants must be a team-player, possess excellent time management skills, and have strong written and oral communication skills.

VIEW JOBS 2/18/2021 12:00:00 AM 2021-05-19T00:00 Under the general direction of Population Health Management, the Behavioral Health Support Specialist will be an integral part the Population Health Management team focused on providing support to patients and physician practices regarding behavioral health management for adults with varying levels of depression, anxiety and substance use. The quality team focuses on an infrastructure that allows us to optimally and proactively manage our populations of patients with chronic conditions (e.g. diabetes, hypertension, depression). This role works collaboratively with the team to implement evidence-based services for primary care patients utilizing tools and training from IMPACT Collaborative Care model and Partners HealthCare, engage patients in iCBT (internet-based cognitive behavioral therapy) and connect patients to community providers when indicated. Principal Duties and Responsibilities: * Ensure systematic screening of adults, using evidence-based screening tools (e.g., PHQ9, GAD7). * Use a variety of educational materials, brief intervention techniques, and community resources to engage patients, increase their motivation to change, and support patients in establishing behavior change goals and implementing plans to meet those goals. * Maintain accurate and up-to-date records and standardized data on all patients. * Manage a target caseload of 80 primary care patients. * Effectively communicate with the patient and distributed care team. * Systematically review the caseload with the consulting psychiatrist and LICSW each week. * Participate in development of maintenance plan with patients to help them maintain a healthy lifestyle. * Works with LICSW on finding community resources for patient when indicated, ensuring patient connection with OP provider. * Attend mandatory trainings relating to work role as scheduled. * Learns and understands Primary Care practice workflows with respect to optimal and coordinated health care for this targeted patient population. * Works within the Population Health Management team and contributes to quality and process improvement efforts when necessary. * Other duties as assigned by manager. * Bachelor's degree, preferably in social work, counseling, health education, psychology, or behavioral health. * Minimum 2 years in relevant setting (health care and/or behavioral health). * Valid driver's license. * Patient-focused experience or customer service background strongly preferred. * Working knowledge or familiarity with electronic health records. * Proven problem-solver and behavioral change strategies with ability to multitask (i.e. Motivational Interviewing and Behavioral Activation). * Comfort with a patient-centered approach and ability to work as part of an interdisciplinary team to improve patient's health and wellness. * Ability to serve as a critical liaison to facilitate connections between people * Ability and willingness to work in a time-limited, structured, and solution-focused environment. * Ability and willingness to function independently and proactively in a primary health care setting. * Ability to maintain a professional stance if conflicts arise with other staff. * Flexibility to adapt to unforeseen needs or circumstances. * Effective use of the Internet and Microsoft Office programs such as Word, Excel and Outlook. * Effective use of electronic medical records once trained.
