Work Shifts ROTATING SHIFTS Job Description 1.

Observes patients physical and psychosocial behavior. Reports observations to Charge Nurse/ Head Nurse. 2. Provides care to assigned patients while reinforcing good health practice.

Ensures that patient's privacy is provided. 3. Assists in monitoring patients for pain screening and report to charge nurse any complaints and responses to prescribed treatment. 4. Monitors patient on constant and close observation according to established protocol. 5.

Documents pertinent information regarding patient's behavior and physical condition as per established guidelines, effectively utilizing the electronic as well as paper medical records, while maintaining patient confidentiality. 6. Prepares patients for examinations by performing assigned procedures such taking vital signs, weights, and assists in the examination process. Collects urine specimen. 7.

Assists patients in activities of daily living. Distribute meals to patients, assist in feeding as needed. Observe food and fluid intake and output and document as needed.

Assists patients with showering and laundry. 8. Participates in providing activities geared towards improving quality of life. Assist in recreational, motivational and socialization activities.

Co-Leads Community meetings, ADL, MICA, Social, and Health groups. 9. Assists in controlling aggressive patients using restrictive means as a last resort. 10. Collects pertinent subjective and objective information, and check patients property upon admission.

Document same and report findings to Charge Nurse/Head Nurse 11. Assists in transporting patients to and from designated areas within the facility in a safe and dignified manner. Also accompanies patients on pass or transfers to outside facility. 12.

Carries out established infection control protocols. 13. Changes linens and make beds. Help in maintaining cleanliness and orderliness of the unit including treatment rooms and patients' rooms. 14.

Anticipates patient's needs • and provide care in a culturally sensitive and safe manner. 15. Maintains competence by attending staff development programs. 16. Conforms to policies governing dress code, attendance and punctuality. 17.

Assists in ensuring a safe and secure environment free of hazards to reduce the risk of injury to all customers. Makes unit rounds and checks whereabouts of all patients, makes environmental rounds. Participates in patient and unit contraband searches.

Assists with the flow of visitors. 18. Communicates effectively with patients, visitors and staff utilizing effective telephone techniques and demonstrating courteous manner at all times. 19. Explains and instructs patients/families concerning the therapeutic regimen. 20.

Attends unit meetings/conferences as assigned. 21. Participates in planning and implementing orientation program for new patients. 22. Participates in community programs such as health fairs, as directed.

Minimum Qualifications 1. Six (6) months of satisfactory experience assisting patients with daily living and therapeutic activities and performing other related functions in a hospital or a health care setting; and 2. Holds or obtains a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA) within sixty (60) calendar days of appointment to the title.
