Community Reach Center (CRC) is an Integrated Health Care and Trauma Informed Care Community that serves a metropolitan area in Denver, CO. Our 600 employees offer a wide variety of services to individuals and families in schools, day-treatment, residential, hospital, and correctional facilities. We are seeking a Family Outpatient Psychiatric Nurse Practitioner to join our Medical Services team who is committed to enhancing lives every day. Under limited supervision this position provides psychiatric medication services at the site assigned by the Chief Medical Officer. In addition, this role provides clinical care, assessment, treatment and support services to adult consumers and /or their families, including planning, implementation and documentation.

Work You'll do: As a Psychiatric Nurse Practitioner, you will:

  • Provide direct psychiatric services through comprehensive evaluation, diagnosis and treatment of consumers. Prescribe and administer medications, assist in consumer medication management in collaboration with the supervising psychiatrist under State of Colorado Guidelines. Stabilize chronic psychiatric illness and treat acute, self-limiting conditions (conditions with a defined diagnosis, predictable outcome and is not life threatening). Collaborate with a psychiatrist to maximize efficiency of psychiatric services. Provide consultations to staff and participate in staff conferences and serve on committees under the direction of the Chief Medical Officer. Provide coverage for mental health emergencies, including mental health holds. Complete medical records and reports and provide need referrals to medical services and primary care providers. Coordinate consumer care regarding access to services, lab tests, case management, primary clinician, outreach services and other agencies. Other related duties as assigned by the Chief Medical Officer. Experience relevant to clinical setting with understanding of psychiatric services in the area of health and psychiatric assessment, clinical diagnosis, psychopharmacology and direct consumer care. Knowledge of psychotropic medication administration and practice. Medical expertise consistent with advanced practice nursing knowledge of the psychotropic and similar drugs, competency in mental health treatment. Provides care to consumers in a manner that recognizes the human potential, is empathic and demonstrates unconditional positive regard while promoting safety, comfort and the therapeutic environment.

Qualifications:

  • Nurse Practitioner degree from an accredited nursing school.

  • Specialty designation and board certification in psychiatric practice.

  • Licensed as a Psychiatric Advanced Practice Nurse in the State of Colorado.

  • Valid DEA license with buprenorphine X waiver.

  • Full Prescriptive Authority in Colorado.

  • Must meet Colorado Medical Board Rules and Regulations for licensure of and practice by Nurse Practitioner.

  • Ability to follow detailed oral and written directions and ability to prepare and maintain records charts.

  • Ability to work with an interdisciplinary team.

  • BLS certificate, every two years.

  • Able, willing and experienced in treating patients ages 6-99 years old.

  • Ability to provide quality care within 20-minute follow-up appointments. Schedule will be Monday-Friday, 8am-5pm (40 hours per week).

  • On-site practice, tele-video is not available for this position.

The Palliative Care Advanced Practice Nurse (APN) provides leadership and oversight in the development and growth of the of the palliative care program. Provides direct and indirect clinical support to patients and families experiencing a chronic and/or terminal illness effecting quality of life. Works in the professional role as an expert clinician, consultant, leader, educator, and translator of research. The APN collaborates with an interdisciplinary team, the referring physician, and coordinates the care of patients and families. The APN ensures the palliative care program is in compliance with legal, regulatory, and clinical policies and procedures. Supplements and support the hospice service line as appropriate.
