Psychiatric Mental Health Nurse Practitioner | Geriatrics

They employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to organizational values, team culture, and mission.We seek a California licensed Psychiatric Mental Health Nurse Practitioner with two (2) or more years of experience  for the role of Psychiatric Nurse Practitioner. The person in this role will provide psychiatric consultation, psychiatric illness evaluation, counseling, and treatment for participants in collaboration with the organization's physicians and community-based PACE physicians. In collaboration with a physician, she/he will perform in-person or telehealth comprehensive psychiatric assessments on participants and collaborate with the interdisciplinary team to develop a comprehensive care plan for each participant. She/he will collaborate with physician(s) and IDT in discharge planning from acute or long-term care settings. She/he will refer participants to a psychiatrist as indicated. She/he will oversee care of participants in the nursing home, making regular visits as dictated by the organization's standards and participant needs. She/he will be a consultant to IDT, which includes communicating participant changes, collaborating on care planning arrangements, and coordinating 24-hour care deliveryCandidates must be a graduate of an accredited school of nursing and be licensed as a Nurse Practitioner in the state of California. Must hold active Psychiatric Mental Health Nurse Practitioner state license. Most hold CDS certification and DEA license. Must have at least two (2) years of experience PMHNP that includes working with the senior population. Due to the role’s work with the geriatric population, proof of vaccination prior to employment is required.The role offers highly competitive compensation (salary up to $185,000 based on experience) and Medical insurance coverage (Medical, Dental, Vision) as well as 17 days of personal time off (PTO), 12 holidays observed annually, and sick time. 401 K savings + match also offered.
