Utilizing FHR's PRISMmodel for the Effective Treatment of Serious and Persistent Mental Illness , assesses and diagnoses patients, provide psychotherapy and prescribe medication management for individuals include chronic mental illness, dual diagnosis, and substance abuse.

PA Certification :Must possess Psychiatric CRNP certification and one or more years of applicable experience.

1.Participates in Agency peer review /clinical risk management meetings as needed.

2.Maintains current state license, state controlled substances license, DEA number and liability insurance.

3.Elicits and records information about patient's medical and psychiatric histories for the purpose of formulating accurate diagnosis and providing relevant treatment.

4.Conducts comprehensive mental status examinations to monitor psychiatric status, formulate accurate diagnosis, and provide relevant treatment.

5.Orders laboratory tests and procedures as indicated to monitor potential side effects of treatment and/or to monitor compliance.

6.Orders assessments or consultations for individuals to assist with diagnostic formulation and/or develop comprehensive treatment plans.

7.Encourages individuals to have routine medical, gynecological, optical, and dental examinations to ensure general health maintenance. Facilitate coordination of care with the individual's health care team.

8.Prescribes and monitors psychotropic medications according to community standards of care and the FHR P&P. Monitors individual's response to pharmacological intervention through individual contact, and collaboration with other members of individual's treatment team.

9.Discusses risks and benefits and obtains informed consent from individual or appointed guardian prior to initiating pharmacotherapy.

  1. Demonstrates ability to integrate general medical concerns into general psychiatric care as evidenced by attention to drug--drug interactions, complications of medical illness, and behavioral effects of somatic illness and treatments.

  2. Provides psychoeducation to individuals and, when appropriate, their significant others regarding diagnosis, treatment options and recommendations, and self-care techniques.

  3. Conducts and supervises individual psychotherapy, family therapy, and group therapy when clinically and programmatically indicated.

  4. Maintains documentation in electronic health record of all individual appointments and telephone contacts in accordance with FHR, CARF, and local Department of Mental Health standards.

  5. E-Prescribe - Utilize e-prescribe feature in CARe to write prescriptions

  6. Prepares psychiatric assessments and progress notes for inclusion in clinical records according to FHR policy.

  7. Attends and participates in treatment planning process and has final approval of treatment plans which is indicated by signature on the plan.

  8. Collaborates and consults with nurses, social workers, case managers, and other practitioners involved in individual's care - both within and outside of the agency.

  9. Performs emergency assessments and refers individuals for inpatient treatment when indicated.

  10. Coordinates individual care with inpatient practitioner through direct communication with inpatient practitioner.

  11. Completes in a timely manner requests to fill out forms made by individuals or outside agencies when indicated such as insurance forms, GPA forms, TDI forms, SSI forms, etc.

  12. Conducts self in a professional and ethical manner in keeping with FHR policy and APA guidelines on ethical conduct.

  13. Prepares papers for court-ordered treatment and testifies in court when indicated.

  14. Supervises, as assigned, licensed clinical practitioners.

  15. Provides on-call coverage for evenings, weekends, and holidays as required.

  16. Participates in FHR committees as assigned.

  17. Completes prescription refills in a timely manner.

Location: Phoenixville, PA Program: Psychiatric Rehabilitation Program-Cornerstone Clubhouse Role Description: In this role the Recovery Specialist I will help each consumer reach a maximum level of self-sufficiency through individualized interventions as well as the support of an overall therapeutic milieu. Interventions are based on service plans developed collaboratively with individual consumers. This program provides supportive services to individuals who have mental health issues living within residential programs, supported-living programs, supported-employment programs, and clubhouse programs. Services vary by program and include life-skills teaching and assistance; work-skills teaching and assistance; promotion of social and recreational activities; and identification, linkage, and coordination of resources. Responsibilities and duties include but are not limited to: * Provide services as defined by the program description, best practice standards and in full compliance with licensure standards and Recovery Model Principles. * Provide assistance to the program coordinator as well as providing assistance/education in areas of medications, budgeting, self-care and supportive counseling to the program residents. * Monitor effectiveness of own service delivery based on outcome measures including Quality of Life Inventory, consumer satisfaction surveys, and treatment/service reviews. As needed, make changes to own service delivery to improve outcomes. * Provide services as dictated by client and program needs. * Develop and implement service plans as per program description. * Develop and maintain linkages with adjunct providers to coordinate consumer services. * Identify resources which are accessible and beneficial to clients and encourage their utilization. * Intervene appropriately in crisis situations. * Provide instruction and hands-on support to promote development of skills identified in service plans. * Support consumers in maintaining healthy relationships and resolving conflicts. * Must be compassionate, flexible and knowledgeable of Supportive counseling. * Must be willing to assist with skill building including "occasional hands on". * Maintain accurate records of billable services provided and submit records in a timely manner as dictated by the program. * Complete other responsibilities as assigned by direct supervisor. Minimum Qualifications: A psychiatric rehabilitation worker shall have one of the following: (1) A bachelor's degree. (2) An associate's degree and 1 year work experience in mental health direct service. (3) A CPS certificate and 1 additional year paid or volunteer work experience in mental health direct service. (4) A high school diploma or GED and 2 years work experience in human services which must include 1 year of mental health direct service. Required Clearances: Pennsylvania Child Abuse clearance (if working in adolescent residential program), Criminal clearance, and Fingerprint based federal criminal history; Valid Driver's License and Verification that employee is not on any Medicaid/Medicare Exclusion list. * Interested persons should complete an online application. * Resume REQUIRED.
