Summary:
The Mental Health Coordinator is a member of the Psychiatry Consult multi-disciplinary team providing care and support to patients and their families with mental health concerns. This position primarily works in the Emergency Department (ED), assisting the ED Clinical Therapists and the ED team in providing patient and family support. Responsibilities include locating placement and coordinating patient transfers, arrangement of outpatient follow up care, contacting the patient's outpatient providers to ensure continuity of care, assisting families with accessing follow up care, and supporting the implementation of patient care plans. Communication across teams and with patients and families is an essential aspect of this position.
Responsibilities:
Optimizes the ED Clinical Therapists productivity by providing clerical support to the ED Clinical Therapists as well as patient/family psychosocial support.
Performs clerical functions including the management of phone lines, faxing, documentation of information received by phone or fax, retrieval of medical records, scanning of documents, and other clerical functions not listed.
Assists with the coordination and management of treatment plans with the patient and the family regarding follow up plans, home care and safety planning (as training allows), community resources/entities, and transitions to other facilities or organizations.
Manages confidential patient information and ensures compliant and accurate management of records.
Functions as a point of contact for the department; responsible for the professional and timely response to calls, messages, and the forwarding of communications to the responsible party.
Hourly Pay: $23.29

Graduate or professional work or advanced degree; or equivalent experience Master's degree in Counseling or a related field Pref

Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements. New hires beginning after 1/17/22 will have 30 days from start date to obtain their BLS certification. Req
