Position Purpose: Perform health plan provider orientations and conduct ongoing educational outreach with a focus on improving quality and financial outcomes within the provider network. Act as liaison between providers and the health plan to enhance the business relationship.

Conduct initial provider orientations as well as ongoing educational outreach

Educate providers regarding policies and procedures related to referrals, claims submission, credentialing documentation, web site education, Electronic Health Records, Health Information Exchange, and Electronic Data Interface

Enhance account relationships by investigating, documenting and resolving provider matters and effectively handling and responding to account changes and correspondence.

Engage providers and educate them on Patient Centered Medical Home initiatives

Perform detailed HBR (Health Benefits Ratio) analyses, Health Information data Information Set (HEDIS) analyses, and create reports for provider Review provider performance by both quantitative metrics and qualitative factors

Create and communicate milestone documents, dashboards and success or improvement metrics

Act as a liaison between the provider and the health plan ensuring a coordinated effort in improving financial and quality performance

Provide information and status updates for providers regarding incentive agreements

Conduct site visits when required

Perform other contracting duties as requested, including but not limited to recommending changes to pricing subsystems, submitting changes to provider related database information and assisting in the completion of special projects

Ability to travel

Education/Experience: Bachelor's degree in related field or equivalent experience. 2 years of combined managed healthcare and provider reimbursement experience.

Advanced knowledge of Microsoft Excel. Clinical or health information management (HIM) experience preferred. Claims processing and/or managed care experience preferred.

Licenses/Certifications: Current state driver's license.

Work Hours/Shifts: M-Th 8AM-5PM, F 8AM-2:30PMHours; M-F Job Summary: The purpose of this position is to provide Advent Health Physician Network (AHPN) subject matter expertise, consultation and education to network providers (market/region dependent) by establishing and maintaining a strong relationship with the provider and practice. Advent Health Physician Network Relations Specialists are expected to make regular visits to provider practice offices, with the intent to advise and educate on a wide variety of topics (examples include population health management strategies and programs, clinical integration, incentive program(s), provider web site, and Population Health Services Organization (PHSO) and provider network policies and procedures.). The Advent Health Physician Network Relations Specialist (AHPN Relations Specialist) is a highly strategic role within AHPN, designed to provide value to the network provider and practice through expertise, consultation, change management, and provider engagement. The Relations Specialist is the relationship manager for the practice and exists to help simplify the provider's experience with the Provider Network and plans while working to identify and address improvement opportunities. As part of the AHPN Team, the Relations Specialist will be responsible for executing plans and delivering key messages to providers and office staff. This position is field-based with the majority of time spent in provider offices. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.p> Knowledge, Skills, Education, & Experience Required: * Demonstrated excellent relationship management skills and ability to work well with others using tact and diplomacy. * Demonstrated excellent negotiation skills * Strong project management & strategic thinking skills * Strong organizational skills * Ability to implement initiatives in a strategic manner with minimal supervision, multi-task, and take direction in a matrixed work environment * Demonstrated experience in preparing and delivering written and oral reports * Strong skills for using various information systems tools to identify performance improvement opportunities, communicate these opportunities to providers and track improvements * Ability to handle difficult and complicated situations and maintain professionalism * Fundamental knowledge of marketing and sales * General knowledge of computer hardware and software to include but not limited to Microsoft Outlook, Word, Excel. * Knowledge of Population Health initiatives and the triple-aim - to include value-based performance incentives for providers (Preferred) * Knowledge of performance metrics (quality and efficiency measures) (Preferred) * Knowledge of CMS' Risk Adjustment model (Preferred) * Electronic Health Record experience (Athena, eClinicalWorks, Cerner, etc.…) (Preferred) * Knowledge of hospital and provider (multi-specialty group, IPA or PHO) operations and services impacted by managed care and health care reform (Preferred) * Demonstrated basic knowledge of processes within the ambulatory practice/provider setting(Preferred) * An understanding of health plan operations and network quality and efficiency initiatives related to providers, including reporting and attestation of quality measures(Preferred) * Bachelor's degree in Business, Marketing, or Health Science or comparable, relevant experience (Preferred) * 3+ years' experience in performance improvement, operations, marketing or relevant physician/hospital/managed care environment. Assist with the development, distributes, and communication of key events and information as directed by AHPN leadership while working alongside CHN counterparts. * Responsible for tracking, measuring, analyzing, and reporting on provider contacts, updating provider demographics, validating provider contact information, and reporting issues in the designated provider reporting tool.
