Overview

Leveraging the foundation of the Patient-Centered Medical Home model of care, the PHM Practice Support Consultant will be responsible for monitoring the health and wellness of primary care practices by monitoring strategic metrics, supporting associated improvement projects and the implementation of population health programs.

Principal Duties and Responsibilities

  • Plan for and lead regular meetings with assigned practices to monitor health and wellness of primary care practice operations with support from manager on strategic approach.

  • Consult with practices about their primary care challenges and process improvement opportunities and offer suggestions and support to achieve improvement goals.

  • Support practices with the development, sustainability and enhancement of the patient-centered medical home model of care.

  • Leverage data to inform discussions and recommend appropriate strategies with guidance and direction from manager to ensure alignment with local RSO goals.

  • Support development and ongoing evolution of the Advanced Primary Care Strategy.

  • Implement Population Health Management programs with support of PHM Workflow Optimization team. Provide ongoing maintenance support for mature Population Health Management programs.

  • Develop and maintain strong working knowledge of Epic functionality, specifically related to workflow development and reporting to support primary care practice needs.

  • Stay informed about national and regional developments related to the Primary Care and Medical Home movements.

  • BS/BA Required.

  • 3 years of relevant work experience.

  • Experience with MS Office applications, including Word, Excel, PowerPoint, and Access.

  • Experience with Epic and other electronic medical records is a plus.

Skills Required

  • Demonstrated ability to move projects forward with support from manager.

  • Ability to foster and maintain relationships with external stakeholders.

  • Solid understanding of the health care environment.

  • Position requires frequent local travel to physician practices.

Working Conditions

Professional office setting located in Assembly Row, Somerville, MA. Requires travel locally to other Partners' sites. Reliable access to transportation to travel between sites for business purposes is required.

Overview Reporting to the Revenue Integrity Manager, the Senior Revenue Integrity Analyst plays a leadership role in a high-profile group tasked with improving revenue results by taking a global view of clinical and financial processes, functions and interdependencies from the provision of patient care to final bill generation. Due to its service focus and project management emphasis, this position requires strong interpersonal and communication skills, superb analytic and organizational skills, and the ability to meet deadlines while influencing, but not directly managing the work of others. PRINCIPAL DUTIES AND RESPONSIBILITIES With responsibility for designated service lines and acting with a high degree of autonomy, coordinates reviews related to Charge Description Master (CDM) integrity. Evaluates current charging and coding structures and processes in clinical departments to ensure appropriate capture and reporting of revenue and compliance with government and third party payer requirements, to ensure consistency across all entities. Analyzes changes to coding and billing rules and regulations by utilizing appropriate reference materials, internet sources, seminars and publications. Executes on work plans to adapt systems and processes to accommodate changes. Formally prepares and presents findings in standard presentation for other committees, including leadership. Assesses the accuracy and build of all charging workflows in EPIC, including documentation, dictionaries, preference lists and other interface or third party charging systems. Provides PHS Enterprise guidance, communication and education on correct charge capture, coding and billing processes to multiple clinical departments and entities. Leads and participates in complex projects related to revenue cycle initiatives. Provides oversight for projects in which support analysts are involved. Mentors and coaches assigned analysts, and assists in oversight and review of analyst's work. Collaborates with Partners eCare(PeC), Revenue Cycle Operations staff, Compliance, Budget Offices, PHS Coding, Internal Audit and other Revenue and Finance departments on revenue management initiatives, across all entities. Develops, maintains and implements Revenue Integrity and CDM Management policies, procedures and training materials. Analyzes EPIC work/error que data and performs root cause analysis. Executes on work plans to correct identified deficiencies. Minimum Seven years of experience in a hospital setting or within the healthcare industry strongly preferred. Bachelor's degree required, Masters preferred. Applicable clinical or professional certifications/licenses such as CPC, AHIMA-CCS, RN, RT, MT, RPH, PMP are highly desirable. Epic healthcare system experience preferred. SKILLS/ABILITIES/COMPETENCIES REQUIRED Ability to research, analyze and interpret healthcare policies, billing guidelines, and state and federal regulations. Full Microsoft Suite applications ability, with strong MS Excel skills required, and MS Access skills preferred. Excellent customer service skills Ability to document clinical workflows impacting revenue cycle. Strong written communication, formal presentation skills, and excellent customer service skills.
