Under the direction of Harm Reduction Program Manager and working closely with the providers at clinics, and Director of Operations Behavioral Health. The Medication Assisted Treatment (MAT) Program Coordinator is responsible for the scheduling and coordination of all patient referrals and follow-up care for the MAT program including completing universal screenings and screening forms related to substance use.

Responsibilities

  • Maintain list of active MAT prescribers and those with dedicated MAT time

  • Maintain up to date MAT patient list

  • Monitor lists for patients that need follow-up

  • Identify inconsistent patients who may need check-ins with coordinators

  • Provide warm hand off to Harm Reduction Peer Support Specialists for support

  • Complete steps of the MAT referral/intake process including:

  • Obtain referral from provider in timely manner

  • Obtain authorization where applicable

  • Provide the patient with MAT program rules/expectations handout

  • Complete universal screenings and screening forms for substance use

  • Schedule MAT pre-intake assessments to determine appropriate fit for program

  • Schedule MAT provider intake/induction

  • Add labs into EMR

  • Ensure screening labs performed prior to the MAT provider appt

  • Communicate appointment information to patient/family

  • Communicate referral/authorization information and pertinent medical information to the specialty provider

  • Follow-up with the patient and specialty provider to document that patient has kept appointment

  • Obtain release of information and request records as applicable

  • Appropriate documentation in the patient medical records

  • Complete Naloxone kit training

  • Seamless transfer of patients to alternative MAT providers when necessary for cross coverage

  • Liaison with local EDs, treatment centers, and correctional facilities.

  • For patients requiring a higher level of care, provide resources for alternative treatment programs

  • Document MAT group attendance in spreadsheet

  • Collaborates and works closely with behavioral health related to MAT program, Harm Reduction program, Street Medicine program, Telepsychiatry, and social and recovery resource assessments and referrals to Complex Care Resource Specialists or to Peer Support Specialists

  • Manages schedules for UCSD Fellows

  • Complete provider requested referrals

  • Document actions taken in the patient medical record in accordance with current clinic, DHS, and Health Plan regulations/guidelines

  • Meet or discuss with patients aspects of the referral process as needed or requested by the treating provider

  • Patient communication in person, by telephone, and/or by mail.

  • Act as liaison between patient, clinic/providers, specialty care providers, health plans/funding sources, and community resources

  • Attend meetings on and off site when deemed necessary by site manager and harm reduction program manager

  • Reviews drug abuse screening test (DAST) and Alcohol Use Disorders Identification Test (AUDIT) reporting daily to identify eligible patients for program

  • Uses appropriate verification techniques to manage changes in project scope, schedule, and costs

  • Reports and escalates issues as needed to management

  • Manages professional relationships with all program staff, community agency partners, and patients

  • Maintain relationships with community-based organizations and treatment centers in the community by touring the facility

  • Other duties as assigned, including verification of insurance eligibility, submission of treatment authorizations, and providing coverage for front office staff

Quality Management and Record Keeping

  • Contribute to the success of the organization by participating in quality improvement activities

  • Monitors and maintains confidentiality in accordance with Neighborhood Healthcare policy and legal requirements

  • Document as necessary in ECW for all care coordination and other services or teaching provided in a timely manner

  • Provides accurate and timely reports to supervisor, as needed

  • Initiates and processes Incident Reports per documentation guidelines

  • Prioritizes activities according to intensity, need, and required follow-up

  • Provides accurate and timely reports to supervisor and site staff as required

  • Maintains professional working relationships with all levels of staff, clients, and the public

  • Respond promptly and with caring actions to patients and employees. Acknowledge psychosocial, spiritual and cultural beliefs and honor these beliefs.

Qualifications

Education/Experience

  • High school/GED required

  • Two years of previous experience within the health-related field required

Additional Qualifications (Knowledge, Skills and Abilities)

  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills

  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form

  • Knowledgeable of medical terminology, computer skills including familiarity with spreadsheets and databases, bilingual (English/Spanish) preferred, strong communication and organizational skills, ability to interact effectively with clinic personnel, patients and outside.

  • Knowledgeable about and experience with issues of mental illness and/or chemical dependency

  • Knowledgeable about and experience with electronic medical record technical documentation

  • Ability to analyze data and create presentations using Microsoft Office Suite

  • Ability to successfully manage multiple tasks simultaneously

  • Excellent planning and organizational ability

  • Ability to work as part of a team as well as independently

  • Ability to work with highly confidential information in a professional and ethical manner

Physical Requirements

  • Ability to lift/carry 10 lbs/weight

  • Ability to stand for long periods of time

Pay Range: $23.31 - $28.74 per hour, depending on experience.
