Project Coordinator (12 Months)

Basic Function and Scope of Responsibilities:

The Project Coordinator, who will fill a full-time position, will support and work with the local health department to provide assistance to families of youth with special health care needs by providing information and referrals for insurance, health services, and community resources. This grant funded program collaborates with the DOH and regional support center to develop resources, access to technical assistance, and identify barriers in accessing health care and supports.

It also provides family information sessions, community engagement, and works with partners to ensure culturally sensitive processes. Additionally, this program supports the transition between youth and adult life, and provides accessible information to families, as well as support meetings and training workshops.

Principal Responsibilities:

● Collaborate with the Department and regional support center (RSC) to develop a comprehensive resource list for Children and Youth with Special Health Care Needs (CYSHCN) and their families.

● Provide timely and relevant information and referrals to insurance, health services, and community resources.

● Support family information gathering sessions and engage in community engagement activities to raise program awareness.

● Work with community partners such as medical providers, childcare providers, and school district representatives to help families access available resources.

● Develop a list of local resources for CYSHCN families with the assistance of the RSC.

● Ensure that communication, outreach, referrals, and engagement strategies are culturally and ethnically sensitive.

● Support youth and young adults with special health care needs in transitioning to adult life successfully.

● Provide families with information on support meetings and training workshops offered by community organizations and the RSC.

Training, Knowledge, Skills, Experience:

● Contractual agreement to obtain a Bachelor's degree in a relevant field such as public health, social work, or a related discipline within 2 years.

● Experience in providing assistance to families, especially those with youth with special health care needs.

● Knowledge of insurance, health services, and community resources related to special health care needs.

● Ability to collaborate with the Department of Health (DOH) and regional support centers.

● Strong communication and interpersonal skills to conduct family information sessions and engage with the community.

● Understanding of culturally sensitive practices and the ability to work with diverse populations.

● Familiarity with the transition process between youth and adult life for individuals with special health care needs.

● Experience in organizing and facilitating support meetings and training workshops.

● Ability to work independently and as part of a team in a grant-funded program environment.

Education Requirements:

● The undersigned party agrees to obtain a bachelor's degree within two years from the effective date of this contract. Failure to fulfill this obligation may result in termination of the contract or other consequences as outlined herein.

Working conditions:

● Contract Promise: The CYSHCN Coordinator agrees to report to the office located in Mineola, NY, at least twice a week, on days mutually agreed upon by the Coordinator and the employer.

This requirement is essential for effective coordination and collaboration with team members and stakeholders. Failure to comply with this provision may result in disciplinary action or termination of employment.

● LHD will participate in quarterly statewide webinars to discuss promising practices; must attend in-person regional CYSHCN meetings; and support family information gathering sessions. LHD will participate in community engagement activities to promote program awareness.

Materials & Equipment:

● Educational materials provided by DOH and Regional Support Center

● Purchase costs of all nonexpendable, tangible personal property. Includes purchase costs of furniture and office equipment such as printers, copy machines, desktop computers, etc. For each type of equipment / furniture requested provide: a description of the item, cost per unit, the number of units, and total purchase cost.

● Some smaller equipment purchases may be recorded as supplies (e.g., fax machines, etc). Check with the Department.
