Position Summary:

This position is responsible for performing a wide range of duties in the Facilities Development Department, be it resource related or Administrative. It includes the implementation & support of all Capital Improvement, renovation, or other projects as assigned related to the Valley Health System. Responsible for communicating with participants, including maintaining updated lists and contact information and responding to and directing requests for information.

Qualifications & Education

College degree or 5 plus years of work experience in a professional office setting. Proficiency in Excel, Word, Access, Power Point and other software programs as required Ability to communicate effectively Ability to work independently with minimal direct supervision.

Ability to work cooperatively within a team environment. Ability to handle frequent interruptions and adapt to changes in workload and work schedule while maintaining composure. Ability to set priorities and respond quickly to customer needs. Good organizational skills required.

VIEW JOBS 10/2/2018 12:00:00 AM 2018-12-31T00:00 RN Transitions Coordinator Full Time Days Position Summary: The Transitions Case Manager serves as a consistent oversight of the Bundle Payment Care Initiative Advanced (BPCI-A), Comprehensive Care for Joint Replacement (CJR) program, and ACO patients within The Valley Hospital. The Transitions Case Manager monitors works to ensure seamless coordination of services throughout the continuum of care. This includes ensuring that beneficiary notification takes place, which the discharge plan is appropriate for the patient based on clinical condition and skilled need for post-acute services, and providing clinical hand-off to Post-acute navigators, home care services, and Population Health Nurse navigators. The Transitions Case Manager will provide oversight, education, and share knowledge regarding internal and external support services and resources. This will include collaborating and coordinating care with the unit-based Case Manager and Social Worker. Will be responsible for facilitating: medication reconciliation, rapid scheduling, coordination of follow-up care, and appropriate hand-off to the next level of care across the continuum. Will identify and assist patients and families in eliminating potential barriers to care by identifying and referring to appropriate resources. Will be an integral part of and professional member of the interdisciplinary team promoting optimal patient care. Qualifications & Education Bachelor's degree in Nursing required. ANCC Case Management Certification preferred. Expectations: Achieve ANCC case management certification within 2 years( effective 2016), or when requirements for sitting for the case management exam are met, as stated by ANCC, whichever comes first, OR graduate of an accredited school of Physical Therapy with a minimum of a Bachelor's degree in physical therapy. Current Registered Professional Nurse in N.J, BSN required, Minimum 3-5 years of clinical nursing practice and experience in case management, experience with, OR current and valid New Jersey license as a Physical Therapist, accountability care organization and or bundled payment program a plus. Candidates must possess excellent interpersonal and customer service skills. Role will require strong problem solving skills with flexible and creative approach to work assignments. Excellent oral and written communication skills required.
