We are looking for a Project Coordinator with MHMG Administration Department at St. Joseph Heritage Healthcare.

Location: Mission Viejo, CA

Schedule: Full-time 80 Biweekly Hour

Shift: 8 Hour Day Shift

Job Summary:

Supports the SCM (Specialty Care Management) team by assisting with clerical and administrative functions related to the care coordination of SCM patients. Responsible for the coordination of Transplant Committee functions. Coordinates care for listed organ transplant patients. Responsible for coordination of SCM program statistics in order to promote quality, cost effective care to SCM patients. Responsible for the training and assistance to staff in multiple St. Jude Heritage computer applications.


  • Knowledge of utilization management, case management provided throughout the continuum of care.

  • High level of proficiency in St. Jude Heritage computer programs.

  • Ability to problem solve as a team member.

  • Ability to function as an advocate for patients.

  • Ability to manage multiple tasks and remain organized.

  • Ability to work autonomously and also as a team member.

  • Excellent communication skills.

  • Proficiency in Excel, Access, Visio and Word computer applications, preferred.

Position Requirements:

Education: Associate's Degree Health care related field.


  • 3 years Experience in utilization management or case management.
  • 3 years Clinical experience in hospital or medical office/clinic setting.

License and Certification: Medical Assistant certification, or Active LVN license.

JOB SUMMARY: Coordinates information flows and oversees loan-level activities and administrative processes associated with the mortgage lending function for the branch and its subdivisions. The Branch Coordinator is the "Quarterback" through which all Field inquiries flow. DUTIES AND RESPONSIBILITIES: * Communicate visibility into loan status for builder partners and title operations. * Provide all required and requested information and documents to Operations partners upon request. * Communicate effectively with Operations partners to manage backlog for multiple branches within the Area, ensuring loans track along the homebuilding process appropriately. * Manage and maintain data integrity for closing dates and closing methods. * Complete all month-end reporting for branch and builder partners and complete any ad-hoc reports upon request. * Assist Branch Mortgage Finance Advisors when needed with administrative tasks. * Maintain all branch matrices and Subdivision setups. * Maintain all branch MICE (Move In Cost Estimate) and HAT (Home Affordability Tool) tools. * Manage Condominium Project Approval processes, as needed. * Complete and issue weekly rate guide to builder Sales Team.
