REM Wisconsin is seeking ahighly motivated, self-starter to serve as a Program Supervisor Float to lead services within a designated serviceor program in the Janesville Area. The Program Supervisor Float will providequality support to assigned individuals and oversee the activities of theemployees within the service or program.

Key Responsibilities:

  • Ensuresthat all people at sites served are regarded with dignity in all interactionswhile providing direct service to assigned consumers including training andassistance in social, behavioral, health care needs and all areas of dailyliving.

  • Assistsin accessing medical and health related services, also assists in thedevelopment and implementation of the client's service plan/behaviorintervention plans for individuals receiving services.

  • Providesdirection, supervision, training and support to assigned staff. Assists in the screening, evaluation anddiscipline of staff.

  • Actsas a mentor for staff to promote teamwork and employee safety

  • Completesand maintains staff schedules.

  • Schedulesand or provides recreational and leisure time opportunities with considerationfor client choice.

  • Providesmonitoring, documentation and reporting in accordance with corporate andregulatory standards.

  • Assistsin the management of client and household funds.

  • Performsdirect care duties.

Qualifications:

Requirements:

  • HighSchool diploma or equivalent and minimum three years' experiencein the human services industry

  • One year supervisory experience,preferred

  • Demonstrated ability to completerequired documentation. Skilled in writtenand oral communications.

  • Validdriver's license and driving record that meets insurability as defined by the

Company's insurance carrier, car registration and auto insurance

Facilitates multidisciplinary meetings (such as Spine Focus Group).# Collaborates with all disciplines of the Neuroscience Institute as well as referring physicians and departments.# Demonstrates self-direction and accountability.# Works closely with mid-level practitioners in Neurosurgery to provide patient consultation regarding treatment plans. Provides leadership and representation for Institute within Mercy and in the community.# Program development is a key responsibility of this position.# # ESSENTIAL DUTIES AND RESPONSIBILITIES: Serves as triage coordinator using patient care pathway protocols/algorithms developed with Medical Director for neuroscience patients. # Conducts patient intakes regarding symptoms and treatment history # Works with Neurosurgery staff to ensure proper diagnostic testing is obtained for Institute patients. # Communicates with referring providers and outside facilities to obtain necessary treatment records. # Communicates treatment recommendations to patients and finalizes treatment plan/appointments with patients. # Maintains close contact with clinical mid-level practitioners in Neurosurgery to provide patient consultation regarding treatment plans to manage emergent patient care issues # Provides advocacy for patients within Institute and Mercy # Communicates with Institute practitioners to update treatment plans # Interacts with Institute providers to ensure and improve patient care quality and efficiency and initiates new procedures # Provides leadership and representation for Institute within Mercy and in the community # Responsible for running of Spine Focus Group (multidisciplinary group of specialists) and Institute meetings # Along with Medical Director, develops policies/procedures for Institute # Responsible for ongoing program development utilizing best practice methods for patient triage and treatment coordination # Coordinates Institute staff development in collaboration with Director, Neuroscience Services. # Leads effort to develop Institute #annual report# with support from Program Specialist # EDUCATION/EXPERIENCE Bachelors Degree required; Masters Degree preferred (degree in nursing, clinical psychology, or other clinical field preferred) 2-3 years experience preferred Previous experience working in clinical setting preferred (i.e. hospital, clinic, etc.). # AGE OF PATIENT POPULATION: ##### _## Neonate# ####_____Child# ######X##### Adolescent# #####X#### Adult# #####X### Geriatric # COMPETENCIES To perform the job successfully, an individual should demonstrate the following competencies: # Quality - Follows policies and procedures. Additionally, this position works closing with the precert/prior auth team and insurance companies to obtain necessary surgical coverage approvals from the insurance companies without delaying scheduled surgeries.
