Position Summary:

The Program Specialist is responsible for fully supporting the Program Manager by assisting with the outreach and in-reach of multiple Roots programs. Will work in collaboration and continuous partnership with Program Coordinators, to provide community outreach in order to link individuals to the Roots Community Health Center for integrated primary care, behavioral health, enabling resources and other supportive services for which they are eligible. This position requires flexibility in hours that may include working evenings and weekends.

Duties and Responsibilities:

  • Provide internal program and direct community outreach to targeted populations.

  • Provide prospective clients with a referral to programs, up to and including setting up appointments at the program site for an intake/assessment.

  • Support Program Coordinator to ensure deliverables fall within applicable scope of work and budget.

  • Act as liaison to the community and assigned Program Coordinator.

  • Provide support in implementation and coordination of activities with internal and external service agencies.

  • Make outbound and receive inbound calls providing detailed information about eligibility and program services.

  • Complete intake documents and evaluates the client's eligibility to receive program services consistent with established guidelines.

  • Maintain effective and timely reports and communication with supervisorial staff to maintain workflow that supports the work process and flow within the organization.

  • Work diligently to generate quality leads through established and approved methods such as client referrals, door to door canvassing, and engagement of community data pull/research of clients served by our organization and word of mouth.

  • Provide Medi-Cal outreach activities at selected community sites to low-income high-risk individuals and families, providing information about services and benefits that the Medi-Cal program offers, assess Medi-Cal eligibility, enroll individuals into Medi-Cal, link clients with health and medical health service providers. For individuals and families who are known to be Medi-Cal beneficiaries, make referrals to connect them to appropriate Medi-Cal covered health services.

  • Assist individuals in understanding and applying for Medi-Cal, including collecting documents and submitting applications.

  • Attend and participate in MAA/TCM implementation training.

  • Attend department and organizational meetings as required.

  • Ability to be on-site and/or in the community full-time.

  • Valid California Driver License with safe driving record.

  • Culturally competent and able to work with people from diverse backgrounds.

  • Strong attention to detail.

  • Excellent verbal, written, and interpersonal skills.

  • Strong problem-solving skills.

  • Ability to multi-task and juggle multiple priorities.

  • Ability to use appropriate organizational skills in setting priorities for work, and a commitment to meeting deadlines.

  • Ability to communicate well at all levels of the organization including working with organization leadership and representatives of partner organizations.

  • High level of ethics and personal integrity; must be able to work with confidential information.

  • Willingness to work a flexible schedule.

  • Consistent attendance track record and prompt arrival to work and meetings.

  • Ability to travel and navigate throughout Roots' service areas and neighborhoods.

  • Frequent walking, standing, stair climbing, bending, stooping, lifting (25 lbs. max).
