Description

Massachusetts MENTOR is looking for an individual who is caringand committed to young children's wellbeing.

Serving as a Program Services Coordinator (Foster CareCaseworker), you can have an amazing impact in the quality of services for thechildren and families we support. Work with children and their IntensiveFoster parents while making weekly visits to their home. Oversee all servicesprovided to individuals and their families, monitor quality of care delivered,and ensure compliance with state regulations. Program Services Coordinators (Foster CareCaseworkers) are a critical link to the community for those we serve, aswell as serving as a bridge between our direct care and clinical staff toensure the seamless delivery of services to the individuals we support.

We are looking for a dynamic and passionate person who is a strongadvocate for children and who is committed to assisting with the challenges ofadjusting to foster homes while working on a permanent placement plan. Our Program Coordinator roles provide a fantastic opportunity to serve familiesand to continue a career path in Social and Human Services.

Specific tasks include:

  • coordinate the development and implementation of services andtreatments to the individuals we serve
  • partner with foster parents and other internal resources tosupport transition needs for children in homes
  • as part of the treatment team, collaborate with many otherprofessionals (Schools Administrators, Funding Sources, Therapists, Guardians,Parents)
  • attend weekly supervision meetings with Clinical Supervisor toaddress questions, gain support, troubleshoot client needs; attend weekly staffmeetings to discuss program needs
  • serve in rotation (with Supervisor back-up) for After HoursEmergency On Call response

What we require of you:

  • Bachelor's degree in human services or related field

  • Valid driver's license in good standing

  • Access to a registered and insured vehicle

  • Successful clearance of background checks including fingerprinting

  • State law requires Social Work Licensing within first year of employment

Documents all actions in a timely manner in an electronic medical record (EMR). Supports other program administration needs as assigned. Responsibilities * Coordinates and schedules home care services to implement the plan of care established by the interdisciplinary team (IDT).      * Works collaboratively with Home Care nurses and other staff, vendors, participants and families to support SE's mission, promote high quality care and service as well as positive internal and external customer relations.      * Communicates with homecare agencies and other vendors to coordinate services including initial orders, changes, cancellations and resumption of services.   * Demonstrates professionalism and acts as an ambassador for Fallon Health and Summit ElderCare at all times  * Emails or faxes open case needs to homecare agencies as required.  * Effectively utilizes established systems and procedures to identify problem cases and alert management of need for additional interventions.     * Follows up on all non-assigned cases as required until services are secured.     * Documents all vendor authorizations and service coordination efforts in the EMR and other identified systems as assigned. * Documents all participant related telephone calls in the EMR.       * Faciliates timely delivery of service authorizations to vendors.       * Maintains a positive working relationship with peers, vendors and other staff.      * Maintains an accurate record of participants' home care services in a desi Fallon Community Health Plan Worcester MA
