Position Summary:

The Program Manager will manage the day-to-day operations of our Youth Adult Community Access Program (YACAP) at PHS' Sexual and Reproductive Health (SRH) Center, a trusted community healthcare provider delivering comprehensive sexual health services to low-income communities of color since 1997. PHS' YACAP will serve Lesbian, Gay, Bisexual, Transgender, Gender Non-Conforming (LGBTGNC) youth and young adults of color, aged 13 to 29, residing in Brooklyn. The Program Manager will coordinate activities related to YACAP deliverables, including program planning, implementation, supervising community wellness advocates, monitoring, and reporting. In addition, the Program Manager will supervise Community Wellness Advocates (CWAs) that will provide health education workshops and tabling around sexual health, including HIV, STIs, HCV, PrEP and PEP at community-based organizations and during community events.

This individual will report to the Senior Program Manager and will work collaboratively with the SRH Centers clinical teams and PHS's Capacity Building Program teams to ensure access to care for program participants and successful execution of all program activities.

Specifically, the Program Manager will be responsible for the following:

Activities planning and Management

  • Coordinate program activities with SRH Centers clinical team to ensure access to care for program participants in a manner that supports health equity and high-quality care.

  • Support creation of procedures, workflows, and metrics to guide and strengthen activities.

  • Work collaboratively with PHS finance team and supervisor to monitor the program budget

  • Work collaboratively with PHS's Quality Improvement (QI) Specialist to implement QI activities related to achieving intended program outcomes

  • Monitor service delivery of grant-related deliverables and propose adjustments as needed.

  • Support with grant reporting requirements, including program and financial reports, and communications with the AIDS Institute.

  • Clear and professional documentation of external and internal meetings through notes and meeting minutes

  • Represent program at grantee meetings, conferences, and public events as appropriate.

  • Ensure ordering and stocking of all medical and program supplies, such as HIV home tests, MetroCards, iPads, and promotional materials

Outreach and Enrollment

  • Travel to local community-based organizations and events, as needed, to support outreach and the delivery of sexual health education.

  • Develop LGBTGNC youth recruitment materials such as flyers, posters, and social media content with input from priority populations, including CWAs.

  • Maintain and develop new relationships with community-based partners to share program details and solicit youth referrals and enrollments.

  • Work collaboratively with Training Manager and Training Coordinator to develop and facilitate 2-week introductory and ongoing training for CWAs.

Team training and supervision

  • Train and supervise up to 3 Community Wellness Advocates (CWAs), representative of the community served (LGBTGNC youth)

  • Co-facilitate monthly meetings with Outreach Coordinator, Health Educator, and CWAs to discuss program progress, troubleshoot any challenges, and identify additional training needs.

  • Participate in quarterly working group meeting to update the larger team on the status and completion of workplan activities.

  • Other duties as directed by supervisor.

Qualifications:

  • Bachelor's degree required, Master's degree in a health-related field preferred OR 5-6 years relevant work experience required.

  • 2 years of experience working on grant-funded projects with a supervisory role preferred.

  • At least 2 years of prior experience working with LGBTQ+ youth and young adults of color

  • Relevant health/wellness training and facilitation experience required.

  • Interest or prior experience in issues related to sexual and reproductive health, reproductive justice, adolescent health, LGBTQ+ affirming care, health equity SDOH, HIV/STI/HCV prevention, motivational interviewing, and substance use and sex harm reduction (or some subset/combination of these) preferred.

  • Attention to detail, as well as excellent time management, organizational, and oral and written communication skills required.

  • Abilities to adapt to diverse audiences and facilitate discussions of sensitive material.

  • Proficiency in Microsoft Word, Excel, and PowerPoint required.

  • Ability to work collaboratively both in person and virtually.

  • Ability to travel locally within NYC.

  • Salary: $70,000.00- $73,000.00.

This position is an on-site position. A cover letter is required.
