Program Manager

  • School-Based Telehealth

Cone's School-Based Telehealth program places a clinical telepresenter in the school who can virtually connect the parent/guardian, interpretive services, and a Nurse Practitioner with the child in the school. The provider uses the Advanced Digital System to examine the child with the parent virtually present. This innovative system called TytoCare allows the provider to triage, examine, diagnose, and treat many medical conditions that would have resulted in early dismissal, absenteeism from school and potentially, a visit to the Emergency Department. The school-based telehealth program provides:

  • A partnership with Guilford County Schools, County Government/ Health Department, school nurses and school staff, philanthropic organizations such as Guilford Education Alliance, and Cone Health.

  • On demand care

  • Point of care testing for respiratory viruses including COVID.

  • Coordination of care with school nurses and primary care practices.

  • And lessens the impact on families for early dismissals for a health complaint.

The Program Manager will provide program support and management oversight of health equity initiatives. This position combines strong project management skills with a working knowledge of health disparity mitigation strategies. A significant part of this position will be to support the engagement of enterprise data analysts and quality improvement leaders and engage external community partners in developing population health initiatives and implementation of specific tactics. The Program Manager for Health Equity will coordinate health disparity mitigation projects and programs, and community-based population health initiatives, and strategic tactics with Cone Health to promote desired outcomes. The PM-HE will pursue and help drive meeting quality metrics. Providing educational instruction and presentations will be integral to the position within and outside the health system. The PM-HE will be a liaison between designated internal and external stakeholders and the

corporation.

Responsibilities

Creates and maintains an overall project tracking MDI board in the Center for Health Equity. This should include, but not be limited to, key performance metrics, drivers of success, and countermeasures.Deploys community population health projects in targeted prototypes and develops strategies for spread and scalability.Facilitates (agenda and planning) the program meetings, including Steering Committee meetings and sub-work group meetings.Leads and supports the program team by teaching, showcasing and supporting all projects.Works closely with the leadership to ensure all programs and updates are aligned for all customers.

Qualifications

EDUCATION:RequiredBachelors' degree in health-related field

PreferredMasters' degree in business or health-related field

EXPERIENCE:RequiredWorking knowledge of quality improvement initiatives, program management in healthcare, proven leadership ability.Proven leadership ability.PreferredLean management systems and A3 knowledge.2 years project management experience.
