Responsible for planning and execution of a personalized activities program, forming and maintaining relationships with community partners, increasing community awareness of PACE.# Must supervise and facilitate Club PACE staff in carrying out the planned activities.# Coordinates volunteer efforts to meet the needs of Total Life Healthcare.# Serves as or designates a representative to function as part of the interdisciplinary team as defined by CMS. # # #
