TITLE: Program Director

GRADE: 15 Exempt

Summary of Position:

Responsible for current program management and assisting in longer term program development including: identification, and recommendations to senior management of future program growth and direction; providing leadership, supervision, development, quality and growth of assigned program or areas; prepare and manage assigned budgets, participate in or conduct contract negotiations, and perform other administrative duties necessary to achieve program(s) or area(s) goals and objectives within the Pathlight, division, external agencies, and assigned program(s) policies, regulations and procedures.

Program Director is the line management position below Division Management/Senior Management with overall program as well as administrative responsibilities. The position typically requires an advanced academic degree suited to the assigned program(s). Program management requires the ability to interpret situations/issues within program policies and independently make decisions necessary to ensure and/or further program leadership, services, and quality. Position is responsible for program results and supervision of associates carrying out program objectives and procedures.

Essential Job Functions:

  • Responsible for ensuring the coordination and monitoring of delivery of services to individuals/families/care providers served to ensure conformity to funding agencies regulations, internal policies, procedures and quality standards, and to applicable program model(s).

  • Oversee, conduct, or participate in or receive needs assessments of those supported and ensure that needs and capabilities are incorporated into service plans; monitor services to ensure that service plans objectives are being carried out; recommend changes or take corrective action as appropriate. Ensure the coordination of major clinical, medical or significant issues for those supported.

  • Responsible for the completion and retention of records, files, documents and reports necessary to operations.

  • Act as prime contact and/or liaison with external business contacts and/or support sources, funding sources, community based supports and others as indicated on behalf of clients and program objectives.

  • Ensure scheduling and staffing according to master schedule staffing pattern or as required for internal personnel and/or externally overseen care providers. Responsible for the development of work teams providing supervision of support staff/external care providers as to Pathlight and program policies and procedures. Recruit and hire staff; train or ensure training of associates/external care providers as appropriate. Participate and/or lead cross agency teams/initiatives. Confer with supervisor and Human Resources, as appropriate, regarding significant matters.

  • Participate in longer term planning and development for program and division as well as administrative work necessary to program. Prepare and/or participate in budget development and other administrative duties internally and/or externally as necessary for program.

  • Write and/or prepare grant proposals for program services/new referrals. Administer activities and budget of awarded grants/contracts. Identify new funding resources and requirements.

  • Supervise line and office personnel. Establish goals and objectives for those supervised; evaluate performance and take corrective actions within Pathlight and Divisional policies and procedures.

  • Coordinate effectively with Nurses and other Clinical Team Members in the provision of services to program members.

  • Emergency on call responsibilities: provide scheduled emergency on-call support as part of the on-call system, which may include direct support to individuals served, if necessary.

  • Contact/Communicate with supervisor or others in more unusual, difficult, or complex situations to obtain direction and/or guidance.

  • Responsible for adhering to regulatory standards as approved CORI Authorizers.

Perform other similar duties as responsibilities necessitate or as directed by supervision.

Knowledge and Experience Required:

  • Advanced knowledge of a professional discipline such as social sciences, human services, business administration. Master's Degree as required by funding source.

  • Over 5 years and up to and including 7 years of progressive learning experience.

  • Working knowledge of computer systems, programs, and applications.

  • Requires valid driver's license and reliable transportation/vehicle required.

  • Eligible for State Licensure.

Skills and Abilities Required:

  • Wide variety of difficult work on assigned program(s) presenting new or changing situations where duties require broad conceptual judgment, initiative and ability to deal with situations and complex factors not easily evaluated.

  • Duties to ensure service plans' objectives are carried out; liaison with external sources; to manage internal or external support providers and similar duties require considerable judgment to work toward overall objectives, and to make decisions based on conclusions for which there is little precedent.

  • Contacts with others within or outside the organization implementing policies and program requiring negotiation, a high degree of diplomacy, and the ability to deal with and influence persons in all types of positions.

  • Works with confidential data such as proprietary program information and future program goals and objectives which if disclosed inadvertently may be detrimental to organization's interests.

The Program Director is responsible for the complete oversight of the Wound Care Center, to include the following functions: day-to-day center operations, staff management, financial management, quality/performance improvement, and community education. The Program Director is also responsible for maintaining collaborative and consultative client relationships within and outside the hospital organization and for creating effective working relationships between Healogics and the hospital. All Healogics employees must perform their job responsibilities according to all Healogics policies, Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable. Program Director Responsibilities: Manages the Center's Operations (20%): Oversees the day-to-day management of outpatient clinic(s) and other wound continuum programs, as applicable. Continuously assesses current clinic flows, processes and procedures, identifies areas of improvement, and develops and implements best practices and appropriate changes to improve outcomes, using the company's resources. Works with hospital and company personnel to ensure availability of adequate resources, supplies, equipment and services. Facilitates the flow of information and maximizes effective communication throughout the program. Prioritizes responsibilities and directs the work of the Center's clinical staff and non- clinical staff. Collaborates with other health care providers, Wound Care Center Providers, and Medical Director regarding clinic and patient needs. Manages direct reports in conjunction with the company and hospital HR departments. This includes interviewing, hiring, motivating, coaching, counselling, establishing performance expectations, and conducting performance reviews. (5%) Performs Financial Management (10%): Manages and coordinates all aspects of the revenue cycle for Healogics and for the hospital partner, as appropriate. Stays current with reimbursement changes, providing physician and staff updates and education as needed. Reviews key financial reports, identifies key indicator trends and develops plans to implement best practices to ensure fiscal responsibility. Tracks and reports all ancillary revenues generated by the program. Manages costs through appropriate utilization and management of labor and supply. Works with Healogics support team to complete financial reviews and presents results to hospital leadership. Manages Community Education/Marketing functions (40%) Works directly with the hospital and Healogics marketing departments to ensure that the wound care program has established strategic marketing plans and works with the same group and the hospital physician liaison to plan, coordinate, and execute activities directed toward increasing patient referrals to the wound care center(s). Works collaboratively, as part of a multi-functional team, to best determine key target referral sources, and develops a systematic approach to build those customer relationships and increase wound care center referrals. Maintains, monitors and updates the list of Physician Practice(s) within a given radius of the hospital on an ongoing basis. Visits Physician Practices and interacts with key physicians to provide awareness of the Wound Care Center program, using educational marketing materials. Also documents all physician office interactions, as well as the visits with the Physician Practices in the Healogics Customer Relationship Management (CRM) system. Communicates on a regular basis with the Area Vice President and the hospital executive sponsor regarding goals, targets, and other relevant referral development information, as needed. Manages Quality/Performance Improvement functions (10%): Implements and manages a continuous Wound Care Center Performance Improvement Program (PIP) and strives to meet Wound Care Center quality indicators. Ensures program is integrated into the partner hospital's PIP program. Participates in hospital committees as appropriate and ensures timely and accurate documentation in the patient record and outcomes database. Monitors patient, referring physician and customer satisfaction. Ensures that Patient Safety Guidelines are followed, that Healogics employees complete annual compliance requirements, and creates an environment that promotes the escalation of compliance concerns as appropriate. Manages Relationships (15%): Maintains excellent relationship with hospital client and continuously seeks to understand needs, confirm goal alignment and demonstrate value proposition. Identifies the hospital strategic goals and objectives and manages the program to achieve those goals, while remaining compliant with all Healogics operations standards. Works effectively and seamlessly at all levels within the partner hospital. Participates in hospital department/management meetings and actively participates in Hospital communication activities. Builds and develops effective working relationships with panel physicians, clinical and support staff. Encourages all program staff to provide excellent customer service to members of other hospital departments. Meets regularly with leadership including hospital and area management Performs other duties as required.
