Join our IDD Team and make a difference in our Tennessee communities! Progressive Behavioral Health Care Company seeks a Program Director to lead, continue the development, and manage the operations of our Program for Individuals with Developmental Disabilities ECF Choices Programs in the Midcumberland region of Tennessee.

This area will serve the cities and surrounding counties of Nashville, Gallatin, and Clarksville, TN. The Program Director is responsible for all facets of the IDD program's operations to include continued program development, implementation, and management, establishing and maintaining effective relationships with other community agency professionals, staff development and management, financial management (developing and maintaining budget), Quality Assurance and compliance, and the overall service delivery of our ECF Choices program serving individuals with Developmental Disabilities. QUALIFICATIONS: Bachelor's Degree in human/social service field required, Master's Degree in human/social service field preferred with at least 5 years experience in a leadership/ supervisor position that includes some experience in developing and managing a budget.

Experience in working with the adult IDD population is required. Comprehensive background check will be conducted. Must be able and willing to travel daily independently throughout above regional area.

The LTSS Svc Coord - Clinician NonRN for our ECF CHOICES Program provides ongoing coordination of ECF CHOICES services and supports with a focus on supporting individuals to explore and obtain competitive integrated employment and maximize community integration. This role supports the member's person centered support plan to meet the member's needs, with the goal of optimizing member health care and promoting and supporting integrated, competitive employment and independent living as the first and preferred option for all individuals with I/DD. * Conducts telephonic or face-to-face assessments for the identification, evaluation, coordination and management of member's needs, including physical health, behavioral health, social services and long term services and supports. * Assists responsible RN for the identification and evaluation of Individuals with Developmental Disabilities who are eligible to enroll in the Employment and Community First Choices Program (ECF) as directed by the responsible RN * Obtains clinical data as needed. * Coordinate and manage member's long term service and support needs with focus on obtaining competitive employment and independent living. * Participates in coordinating care for members with chronic illnesses, co-morbidities with the health care team, to ensure cost effective and efficient utilization of health benefits. * Assists the member in establishing short and long term goals through development of the person centered support plan and empowers members supported to fully engage in the planning process. Decision making skills will be based upon the current needs of the member and require an understanding of disease processes and terminology and the application of clinical guidelines but do not require nursing judgment. Requires: * LPN/LVN, LCSW, LMSW, or LPC or license other than RN in accordance with applicable state law; 2 years of experience in working with individuals with chronic illnesses, comorbidities, and/or disabilities in a Service Coordinator or similar role; or any combination of education and experience, which would provide an equivalent background. * Current, unrestricted LPN/LVN, LCSW, LMSW, or LPC or license other than RN in the state of Tennessee. * May require state-specified certification based on state law and/or contract. * Day travel (70% of the time) throughout assigned territory * Experience with Microsoft products strongly preferred. * Experience working with Individuals with Intellectual and Developmental Disabilities.
