Job Summary The Pulmonary Technology Dependance Program(PTDP) Program Coordinator provides assessment, planning, facilitation, and advocacy for optimal clinical and financial outcomes. In collaboration with the attending practitioner, the family and other members of the health care team, coordinates the individualized plan of care and communicates the plan to appropriate stakeholders. The PTDP Nurse assists with appropriate use of resources both within the facility and external to the facility. Communication and collaboration may include coordinating admissions with referring facilities and providers, utilization review, discharge planning, transportation, and care coordination. The PTDP Nurse ensures that high quality care is provided as efficiently and cost effectively as possible. Education Graduate of an accredited school of nursing Experience Four years of nursing experience required. At least one year in pediatric nursing is highly preferred. Nursing experience may include inpatient or outpatient nursing, case management, utilization review, discharge planning, community health nursing. Utilization review, and/or discharge planning experience desirable. OR MSN degree with less than one year of recent nursing experience, preferably in pediatrics, preferably outpatient setting Management experience preferred Licensures, Certifications, and/or Registries Current RN License in the State of Alabama Current BLS certificate required: BLS Instructor or obtained within one year of employment National Certification preferred. Pediatric certification, or other Case Management/Care Coordination certification is preferred. In some instances, a certification may be required within a specified time frame.#

Education
Graduate of an accredited school of nursing
Experience
Four years of nursing experience required. At least one year in pediatric nursing is highly preferred. OR
