You will handle the day-to-day management of the Caregiver Respite grant, Information and Assistance case management special needs program , grant applications, and record retention to enable the Senior Services' Program to serve current and new clients more effectively. To succeed at this role, you will need not only be excellent at completing key tasks, but also contribute to the long-term success and strategy of the practice.

Salary Range: $27.14 - $36.72 / hour

Essential Required Tasks/Examples of Duties
Maintains confidentiality of information consistent with applicable federal, state and county rules and regulations.
Maintains awareness of federal, state, and local laws, rules and regulations pertaining to Health & Human Services operations.
Coordinates all aspects of the Community Resource and Outreach Program in the Broomfield area.
Works with other non-profits and businesses to secure resources and support for education, information, and referral for the Broomfield senior community.
Researches, writes grants, and maintains DRCOG grant compliance for the Caregiver Respite program.
Provides information, referral, and support to elders and their families in Broomfield.
Coordinates with other agencies to assess community needs, determine gaps in services, and develop plans for service provision.
Provides consultation and information and referral for frail elderly citizens (those unable to make decisions or who do not have family support systems to assist with decisions). Schedules and conducts home visits for the purpose of evaluating client needs.
Assists frail elderly through discussions with individuals and their family members to determine appropriate service agencies to meet their specific needs (such as health, financial, emotional, safety/crime, social security benefits, housing, legal redress, nutrition and transportation).
Develops individual plans for care and service delivery; contacts or assists clients in contacting selected service agencies; follows up with clients to ensure needs are met; maintains client records; performs outreach to identify and serve low-income, handicapped, minority and isolated elderly; makes appropriate referrals to Single Entry Point Case Manager and Adult Protection Service Caseworker. Gathers and analyzes data and prepares reports on the needs of the elderly and services available to meet those needs.
Maintains/updates Community Resource records and materials; prepares statistical and program reports as required.

Any equivalent combination of training and experience that provides evidence that the applicant possesses the Necessary Applicant Traits.
NECESSARY SPECIAL REQUIREMENTS: - Must be at least 21 years of age. Must possess and maintain a valid driver's license and safe driving record for continued employment. Must obtain valid First Aid and CPR Certifications within 90 days of hire and maintain these certifications for continued employment.
New hires must, as a condition of employment, pass the following pre-offer and post-offer/hire processes: oral board; reference checks; background checks including police check, sex-offender registry check, and child abuse registry; driving check through DMV; and CAPS background check.
