The Program Coordinator, CMS Intervention Program grant will help implement and coordinate a public health research project under the supervision of Manisha Parulekar, MD (Principal Investigator). Currently, there is one ongoing research project funded by the Centers for Medicare & Medicaid Services (Hackensack University Medical Center Accountable Health Communities Track 2 Assistance Intervention: Improving Population Health in Bergen County, New Jersey). The Program Coordinator for this grant will play a critical role in ensuring access to health and social care resources for community-dwelling beneficiaries with health-related social needs.

1.Support Program Manager in departmental process improvement efforts across clinical delivery sites related to the project. 2. Coordinate and monitor implementation of all community service navigation services standard operating procedures to ensure program fidelity. 3. Work with program staff to provide community-dwelling beneficiaries with health-related social needs access to resources. 4. Conduct periodic service gap analyses and updating a community resource inventory with partners to assure that resources are available in the community to meet the health-related social needs of beneficiaries. 5. Represents the project at various community organization/coalition meetings. 6. Conduct training sessions for screeners and navigators throughout the course of the project. 7. Administer patient surveys, data entry, tracking, and monitoring of patient appointments. 8. Act as a liaison between hospitals, primary care providers, specialists, community resources and managed care/insurance plans on behalf of patients to ensure person-centered coordination of care. 9. Responsible to ensure that data collection and reporting is accurate and complete. 10. Assist with developing all program progress reports, presentations, and documents. 11. Reports on project activity and progress regularly with the Principal Investigator and Program Manager. 12. Reports and escalates project issues to management as needed. 13. Assist with supervision and support of assigned staff, which will include Community Health Workers and Interns, and monitoring of navigation cases and appropriate service delivery. 14. Other duties as assigned by Principal Investigator and/or Program Manager.


Education, Knowledge, Skills and Abilities Required:
1.Bachelor's degree in Human Services, Social Work, Public Health or other social or behavioral science related field. 2. Minimum 2-4 years of social services, human services or community health experience or the equivalent. 3. Minimum 1 year staff supervision experience preferred. 4. Requires the ability to travel to multiple office locations; personal vehicle and automobile that is insured. 5. Demonstrates ability to carefully set priorities, meet deadlines, and schedule time efficiently. 6. Ability to accurately type and effectively operate a computer and use a variety of common software programs including Microsoft Office. 7. Ability to follow-up and follow-through with strong attention to detail. 8. Ability to work well and maintain professionalism under occasional times of stress and pressure. 9. Demonstrates ability to follow oral and written instructions. 10. Demonstrates ability to work with other people through a cooperative effort. 11. Possesses strong interpersonal skills as demonstrated by compassionate, courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, external business partners, and the community.

This position focuses on expanding knowledge and skills, provides effective direct care as part of the interdisciplinary team to a variety of complex patients. This position directs and oversees the care provided by other caregivers through delegation, validation, evaluation, and coordination. This position effectively coordinates patient care/unit activities among nursing peers, physicians and support services. Responsibilities 1. Demonstrates familiarity with the shared governance model and the blue print for professional nursing. 2. Consistently and independently prioritizes and delivers dependable and effective patient centered care in a variety of patient situations, from common to complex utilizing the nursing process. 3. Performs ongoing assessment by demonstrating refined assessment skills, integrating appropriate data sources. 4. Plans individualized patient care based on assessment and implements plan to meet immediate needs in collaboration with the interdisciplinary team. 5. Evaluates patient's response to interventions and adjust the plan of care accordingly. 6. Accurately documents the components of the nursing process per HUMC policy in a timely manner. 7. Demonstrates ability to communicate effectively by identifying barriers to communication and seeks assistance in facilitating communication. 8. Identifies ethical issues in the practice setting and brings them to the attention of other team members. 9. Utilizes clinical policy/procedures as well as other practice references on HUMC intranet as resource to deliver proper patient care and achieve desired outcomes. 10. Discusses care as it relates to evidence-based practice. 11. Recognizes and responds to opportunities to promote and market HUMC. 12. Recognizes and responds to opportunities to enhance patient satisfaction. 13. Adheres to American Nurses Association Code for Nurses with Interpretive Statements. 14. Identifies the needs of patient population served and modifies and delivers care that is specific to those needs (i.e. age, culture, hearing and/or visually impaired, etc.). This process includes communicating with patient, parent and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.). 15. Adheres to the standards identified in the Medical Center's Organizational Competencies. 16. Solicits and offers feedback on patient concerns and practice setting issues. 17. Demonstrates a developing leadership role by accepting the role of charge nurse. 18. Actively participates in practice setting management including but not limited to performance improvement, projects and committee participation. 19. Demonstrates ability to identify conflicts and resolve them with assistance. 20. Precepts the newly employed staff and students in compliance with established Preceptor Program standards. 21. Collaborates with Clinical Education to review mandatory education and annual competencies based upon accreditation, regulatory and HUMC requirements. 22. Accepts accountability for professional development. Qualifications Education, Knowledge, Skills and Abilities Required: 1. Graduate of a NLN/AACN accredited program in nursing. 2. BSN; or minimum of 1 year experience for existing, internal RN candidates only. 3. Minimum of 1 year prior nursing experience.
