Job Summary:

CareNow Urgent Care is seeking experienced, PRN board certified physician assistants or nurse practitioners to join our Nashville, TN clinics.

Qualified Candidates:

  • Comfortable seeing ages three months and up

  • Must have three or more years of experience with the ability to work independently

  • Board certified

  • Has or willing to obtain Tennessee state medical licensure

  • Ability to perform first reads on x-rays

  • Skills to perform patient treatment procedures for most acute illnesses or injuries to include: pediatric -and adult care; I&Ds; suturing, etc.

If medical and/or religious accommodation requests are denied, you must take steps to be fully vaccinated, or your employment will be terminated.</p><p>Job Description Summary:</p><p>The Clinical Transition Educator is responsible for providing clinical education and training to ensure successful transition of patients to an Option Care Health care delivery model.</p><p>The Clinical Transition Educator will also be responsible for partnering with the sales team to increase the number of patients being transitioned to OCH care delivery.</p><p>Job Description:</p><p>Job Responsibilities</p><ul><li><p>Evaluate, educate, and train patients, caregivers, and facility staff about how OCH services &amp; products will be facilitated in an alternative site, in-home, or virtually in order to ensure successful transition of patients to an Option Care Health delivery model.</p></li><li><p>Conduct patient assessments and evaluations to determine patient viability to join the OCH care delivery model.</p></li><li><p>Partner with, and coordinate with the OCH sales team to understand clinical transition educational needs, and to develop improvements aimed at increasing patient transition volume.</p></li><li><p>Maintains confidentiality of patient and proprietary information and observes legal guidelines for safeguarding the confidentiality of patient and proprietary Option Care information.</p></li><li><p>Provides proper documentation of education utilizing OCH applications and technology.</p></li></ul><p>Supervisory Responsibilities</p><p>Does this position have supervisory responsibilities? NO</p><p>(i.e. hiring, recommending/approving promotions and pay increases, scheduling, performance reviews, discipline, etc.)</p><p>Basic Education and/or Experience Requirements</p><ul><li><p>Active and unrestricted Registered Nurse (RN) license required.</p></li><li><p>Minimum of 2 years of experience in the healthcare industry.</p></li></ul><p>Basic Qualifications</p><ul><li><p>Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors.</p></li><li><p>Experience applying knowledge of standard practices for all services offered as well as current relevant and applicable standards (i.e. ACHC, URAC standards).</p></li><li><p>Experience providing customer service to internal and external customers, including meeting quality standards of services, and evaluation of customer satisfaction.</p></li><li><p>Basic PC skills: Able to competently use internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint</p></li><li><p>Experience in identifying operational issues and recommending and implementing strategies to resolve and improve processes.</p></li><li><p>Access to a reliable means of transportation which will enable the incumbents to travel to care facilities, home visits and multiple hospitals.
