GENERAL PURPOSE OF POSITION: Assess and counsel clients regarding psychological or emotional problems and develops and implements therapeutic treatment plans and discharge safety plans.

Educate families and clients regarding diagnoses and safety planning. Develop, implement, and administer therapeutic treatment plan by performing psychosocial assessments and compile all pertinent documents to facilitate treatment plan. Participate in treatment team sessions with completion of all monthly treatment plan reviews.

Observe patients for abnormal physical or mental behavior and monitor progress with components of treatment plan. Conduct and document individual and group therapy sessions according to treatment plans. Contact and schedule counseling sessions with family, significant others, and involved agencies.

Arrange for family visits off and on facility sites. Act as liaison with family and other agencies to reunify patient with family and reintegrate back into community. Perform daily tasks of counseling by instructing patients in progress charting.

Respond to any emergency situation involving patients in treatment. Participate as a team member by cooperating with staff and demonstrate compromise and expression of opinions as appropriate. Attend staff meetings as required and perform duties as a positive role model for patients and colleagues.

Utilize chain of command in all aspects of program and follow organizations policies and procedures. Perform community relations by ensuring all written and verbal information is clear, concise, legible, and accurate. Communicate and cooperate with families, relevant external personnel, and staff members. Document all external and internal contacts and relay patient information according to company and governmental guidelines maintaining appropriate confidentiality.

Individual must posses a Master's Degree in Psychology, Social Work or Mental Health Counseling and have the following:

Position tasks and responsibilities include: * Plans and uses therapies involving physical exercise, massage, heat, water, light and electricity. * Utilizes various mechanical and electrical equipment and prosthetic and orthotic devices. * Evaluates, records and reports on patient's response to treatment and progress toward identified goals. * Integrates physical therapy treatments with other aspects of patient care. * Assists patients reach their maximal reasonably levels of function and to cope with their limitations. * Monitors, records and communicates patient condition as appropriate utilizing computerized documentation systems. * Instructs and educates patients and families. * Participates in patient discharge planning. * Provides age and culturally appropriate care. * Orients and mentors new staff members. * Follows Standard Precautions using personal protective equipment as required. Required Job Qualifications include: * Graduate of an accredited school of Physical Therapy. * B.S. or M.S. in Physical Therapy. * State licensure as a Physical Therapist. * Current certification as a BLS Healthcare Provider. * Customer service abilities including effective listening skills. * Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment. * Ability to perform work that requires frequent standing, bending, reaching, squatting, kneeling, moving, lifting of patients and/or equipment up to 50 pounds.
