Responsibilities

Responsible for providing direct patient care in accordance with physician orders, recognizing standards of practice and establishing company policies and procedures. Responsible for following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Follows the plan of care, provides treatments, and works collaboratively with the members of the team to help meet positive patient care outcomes. Provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care. Implements current nursing practices following a comprehensive assessment and plan of care. Provides accurate and timely documentation of patient services to reflect the plan of care. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self-plan of care. Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group, including Hospice Aide, Director of Clinical Services Hospice, and Chaplain. Uses and prepares equipment and supplies effectively, adhering to aseptic techniques. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. Participates in personal, professional growth and development. Also participates in organization's quality assessment performance improvement teams and activities.

Qualifications

Education and Experience Graduate of an accredited Registered Nursing program. Minimum of one (1) year experience as a Registered Nurse preferred. Complies with accepted professional standards and practice.

Certifications, Licenses, and Other Special RequirementsCurrent State RN License. Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.

Management/Decision MakingUses limited independent judgment to make decisions based on precedents and established guidelines. Solves problems using standard procedures and precedents. Knows when to refer issues to supervisor and when to handle them personally.

Knowledge and SkillsHas a working knowledge of a skill or discipline that requires basic analytic ability. Has an overall understanding of the work environment and process. Has working knowledge of the organization. Expresses verbal and written communication in a clear, positive and cooperative manner. Demonstrates effective organizational skills. Understands principles of pain/symptom management. Must be able to function in a stressful work environment. Ability to market and deal tactfully with customers and the community. Ability to learn the hospice regulations and requirements, governmental regulations and CHAP standards.

Responsibilities As primary individual accountable for coordinating the patient centered care for the healthcare team, is responsible for providing all services in accordance with the Nurse Practice Act, federal, state and local laws and within the guidelines of his/her professional organization. Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary. Responsible for the planning, coordinating and delivery of quality interdisciplinary home health care services. Compliance with company standards and federal/state guidelines will be maintained at all times. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Director/Director of Professional Services or the Clinical Supervisor/Assistant Director of Professional Services and assumes responsibility to coordinate patient care for assigned caseload. Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes and outcomes as appropriate. Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.
