POSITION SUMMARY: This position delivers professional nursing care to patients in accordance with the established plan of care and Company policies and procedures. Provides care consistent with all relevant professional standards and state and federal requirements

ESSENTIAL DUTIES:

  • Assess, plan and intervene using the nursing process to meet the patient's health care needs.

  • Observe and report any signs or symptoms indicative of changes in condition of the patient and family situation. Document such reports and observations in the clinical record and notify supervisor.

  • Perform assigned nursing procedures, including the administration of treatments and medications in the care of the patient, in accordance with the State Nurse Practice Act and the plan of care.

  • Teach the patients/caregivers appropriate self-care techniques.

  • Notify the Case Manager, physicians, and families of changes in the patient's condition.

  • Perform LPN and Home Health Aide supervisory visits, in accordance with state regulations, to evaluate competency and provide appropriate instructions.

  • Maintain data collection, recording, and reporting systems to ensure proper service, uniform accounting, data collection, and measurement of outcomes.

  • Communicate potential risk management issues and report abuse or unsafe/illegal practice as required by law.

  • Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned.

  • Work with patient, family and health care team members effectively to coordinate patient care.

  • Participate in the on-call schedule and weekends/after hour visits, as necessary.

  • Maintain a high degree of confidentiality at all times due to access to sensitive information.

  • Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.

  • Follow all Medicare, Medicaid, and HIPAA regulations and requirements.

  • Abide by all regulations, policies, procedures and standards.

  • Performs other duties as assigned.

POSITION REQUIREMENTS & COMPETENCIES:

  • Graduate from an accredited registered nursing program

  • Licensed as a RN in the state of practice

  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing

  • Home health experience preferred

  • Ability to make home visits and provide nursing care in the home environment

  • Ability to complete necessary documentation appropriately, legibly and timely

  • Interpersonal, verbal, and written communication skills necessary to complete job responsibilities

  • Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation

VIEW JOBS 10/7/2020 12:00:00 AM 2021-01-05T00:00 The Hospice Admissions Registered Nurse, will report to the Patient Care Manage and is a member of the Interdisciplinary Team. This position performs pre-admission evaluations for appropriateness for hospice services. Initiates the admission process by educating and informing patient/family of hospice and its services. This position also obtains informed consents, provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life. Essential Duties: * Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and initiate intervention by other members of the interdisciplinary team * Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care * Provide regular visits to re-assess the patient's needs and update the plan of care * Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician * Perform clinical pronouncement of death and provide support to family * Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely * Report any changes in patients' condition, patient needs, issues or problems, immediately to attending physician, nursing supervisor and interdisciplinary team, patients' family and other authorized agency personnel * Provide teaching to family, caregivers and interdisciplinary team members to ensure continuity of patient care * Develop and maintain familiarity with Federal, State, Company policies and procedures regarding patients benefit * Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines * Share in providing 24 hour, 7 day per week coverage to patients and families as necessary * Completes an initial, comprehensive and ongoing comprehensive assessment of patient of family/caregiver to determine hospice needs * Develops a plan of care that identifies interventions which will allow patient and family/caregivers to meet their goals * Ensures that equipment, supplies, medications and other necessary items and ancillary services are available and delivered/provided when needed * Other duties as assigned PERFORMANCE RESPONSIBILITIES: * Maintains positive internal and external customer service relationships * Maintains open lines of communication * Plans and organizes work effectively and ensures its completion * Meets all productivity requirements * Demonstrates team behavior and promotes a team-oriented environment * Actively participates in continuing education programs, staff meetings, conferences, agency in-services and community events as requested/required by their supervisor * Represents the organization professionally at all time Position Requirements & Competencies: * Graduate from an accredited registered nursing program * Licensed as a registered nurse in the state of practice * Must possess current CPR, First Aid Certification * 1 year Hospice experience, preferred * Must possess the ability to make independent decisions when circumstances warrant * Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and general public * Must be knowledgeable of quality assessment and assurance procedures * Must pass completion of pre-employment health screening, drug screen, background check where applicable by Federal and State laws, company policies Working conditions: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
