Wellstone Regional Hospital is a 100-bed acute care facility located in Jeffersonville, Indiana and has been providing quality health care to the residents of Southern Indiana and the Louisville area since 2003. Wellstone specializes in the treatment of Adolescents, Children, Adults, CD, and the Geriatric population.

In addition to our in-patient services, we offer out-patient programs as well. Wellstone currently has a LPN PRN position available. The candidate selected will work with all populations.

The LPN performs under the direction of a Registered Nurse in providing nursing care to assigned patients, utilizing basic understanding of psychiatric and personality disorders. Ideal candidate will exhibit Service Excellence at all times; will be professional, work as a team, and will treat all co-workers and outside vendors as guests. Must be able to multi-task and have effective communication skills.

PRN's have a flat rate; will also receive shift differential. This position is not eligible for benefits; however, there is a lot of flexibility with scheduling. Must commit to 4 shifts per month (3 of those being weekend shifts.) One winter and one summer holiday required.

QUALIFICATIONS

Education: Graduate from an accredited professional school of nursing.

Experience: Minimum one (1) year as a LPN in psychiatric nursing preferred.

Licensure: Licensed to practice vocational/practical nursing from the Indiana State Board of Nursing.

Additional Requirements:
Must pass a medication administration exam before administering medications. CPR/First Aid certification and successful completion of CPI, including restraint and seclusion procedures, and prior to assisting in restraining process.

Hours of Work: 8am-8:30pm; and other times as indicated by immediate supervisor.

Re-assesses residents when appropriate and updates care plans when needed. * Follows a cyclical schedule as related to the RAI and care planning process ensuring adherence to strict federal timelines. * Assists as directed, interdisciplinary care plan meetings and conferences with Social Services, family members, and residents to encourage/facilitate family and/or resident participation and ensure positive resident outcomes including the attainment of the highest practicable physical, mental, and psychosocial well-being. * Assists in developing plans of care by eliciting input of staff and, to the extent practicable, the consent of the resident and/or resident's interested family member, legal representative to include measurable objectives and timetables and accommodate resident's needs and preferences including choices about preferred intensity of medical treatment. * Maintains consistent and effective communication with clinical departments and other disciplines regarding resident admissions, RAI schedule changes, as well as, resident change of condition and responses to interventions to facilitate suggestions of alternative approaches and revise and update plans of care to reflect progress towards goals. * Enters data into computer system as directed by MDS Coordinator. * Assists in conducting quality assessment and assurance activities, including regulatory compliance reviews to monitor performance and to continuously improve quality. * Actively participates in daily quality and regulatory compliance, lending to continual survey preparedness. * Assists in the development of corrective plans/actions as needed or identified by ASC representatives, internal QA&A, and regulatory agencies. * Assists the Director of Nursing in the daily functions of clinical services as directed by the MDS Coordinator.
