Location: Kansas City, MO

Hours and shift: PRN (as-needed)

JOB DESCRIPTION

REQUIREMENTS

Currently licensed to practice nursing in the state of hire

Current CPR certification

This position requires a background check

Under general direction of the Clinical Manager, works in the home care setting for assigned patients according to the care plan established by a registered nurse. Demonstrates self-directed behaviors acting on results of information obtained. Maintains schedule to meet both client and agency needs. Demonstrates understanding/accuracy in all aspects of patient care. * Information Management: Documents provision of care/treatment according to professional practice and regulatory guidelines; informs Case Manager of the health care team on any issues impacting the plan of care, in an organized and timely manner. Protects the confidentiality of the client care information. Demonstrates proficiently with Agency documentation. All information entered and synchronized on a daily basis. * Professional Development: Attends educational offerings to expand/update knowledge base; shares information with team members to improve provision of care; participates in activities to improve/enhance the operations of the agency; maintains license(s) and CPR certification. * Flexibility: Adapts behavior and work methods in response to new information, changing condition or unexpected obstacles. * Demonstrates courtesy, empathy and respect for the variety of cultures, beliefs, and heritage of all individuals, patients, families, co-workers, physicians and volunteers. Maintains positive work relationships and contributes to a team environment consistent with the agency's Blueprint for Success Program (core values, code of conduct, etc.). WHAT DO YOU NEED FOR THIS POSITION? You will be a graduate of an accredited school of nursing and IV Certified. Have a minimum of one year of recent experience in an acute care setting or community health preferred. Missouri and Kansas LPN Licensure and CPR required.
