As a Registered Nurse (RN) or Licensed Practical Nurse (LPN), you will render professional nursing care to patients in their home by assessing, developing, implementing, and evaluating home nursing care needs of assigned patients. You will also educate and advise your patients and their families in how to get the most from their treatment programs.

Essential Functions

  • Provide skilled nursing interventions according to the Plan of Care in the treatment of the patient/clients illness, rehabilitative needs and preventative care. Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.

  • Consult with the RN Supervisor regarding needed changes in the Plan of Treatment. Accept verbal orders from physician where permitted by state law/regulations/Nurse Practice Act and communicate these orders to the RN Supervisor.

  • Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team in a timely manner regarding patient/family needs and status changes.

  • Participate in report, care coordination activities/ case conferences and discharge planning..

  • Maintain the highest standards of professional conduct by delivering care in a manner that protects and preserves the patients dignity, rights, values, beliefs, privacy and autonomy

  • Complete a clinical note for each visit within required timeframes, capturing physical assessment, home environment, medication changes and any skilled interventions provided during the course of the visit as well as any care coordination with health care team. This note will be incorporated into the patient's record

  • Participate in all required in services programs

  • Participates in educational offerings and clinical experiences offered by LHC to maintain current knowledge, skills, abilities and judgment in clinical practice

  • Adhering to and supports the agency's care management model

  • Participating in Quality Improvement activities

Required Skills:

  • Graduate from approved LPN program or Associate Degree or higher in Nursing

  • Must have current Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure for state of TN or compact state licensure

  • Current drivers license, vehicle insurance and access to a dependable vehicle

  • Ability to successfully complete required background check and drug screen

  • Current CPR certification required

He/she demonstrates skill in developing and guiding collaborative collegial practice that delivers safe competent care based on meticulous application of the nursing process. He/she demonstrates sensitivity and understanding of the diverse cultural backgrounds of patients and caregivers. The position involves maintaining current knowledge of evidence based nursing practice and the ability to demonstrate critical thinking skills in the implementation of all phases of the nursing process. He/she actively participates in patient care quality and performance improvement through data collection, aggregation and analysis of relevant key indicators of quality (KIQ's). This commitment to quality results in the development and implementation of action plans to include the monitoring of the effectiveness of improvements. The Shift Leader assists with patient assignments, planning, organizing, evaluating, and supervising nursing care on the zone. The Shift leader provides direction and support for nursing staff. The Shift Leader serves as role model for clinical and ethical practice on the zone. The Shift leader is responsible for education and implementation of patient centered care initiatives. The Shift Leader will take partial/full patient assignments as dictated by the needs of the department. Individual must possess excellent interpersonal communication skills and function as a liaison between physicians, interdisciplinary treatment team, patients, and families. It is of the utmost importance that written communication is legible. This individual serves as resource and clinical expert for mentoring staff, team development, and nurse retention. The Shift Leader is expected to set an example for all employees of commitment and loyalty by openly supporting and participating in hospital projects and events. The person in this position must be able to work as a team leader by communicating effectively with team members and providing an open dialogue for solutions. The person must be committed to continuing education by staying abreast of current medical/healthcare information and by applying evidence-based practices in areas of responsibility in order to remain competent in the ever changing role of the position and development in health care. Teaching colleagues/peers and serving as an effective role model is a basis of professional practice. The Shift Leader demonstrates compliance with established MSHA policies, dress code, and standards of professional conduct.
