Overview

Licensed Practical Nurse (LPN) - Private Duty - Full-Time

Location: Newbern, TN

Job Overview: Adoration Health is seeking a dedicated and experienced Licensed Practical Nurse (LPN) to provide high-quality private duty care in Newbern, TN. This full-time position involves working night shifts and requires expertise in G-tube, trach, and vent care.

Key Responsibilities:

• Deliver specialized nursing care, including G-tube, trach, and vent management.

• Administer medications, treatments, and medical procedures as prescribed.

• Monitor and document patients' conditions and progress.

• Communicate effectively with physicians, patients, and family members.

• Educate patients and their families on health care and self-management.

• Maintain accurate patient records and ensure compliance with HIPAA regulations.

Schedule:

• Full-time hours

• Sunday to Thursday, 9 PM - 7 AM

Qualifications:

• Active LPN license in the state of Tennessee.

• Minimum of 1-year nursing experience, preferably in home health or private duty.

• Extensive experience with G-tube, trach, and vent management.

• Strong clinical and assessment skills.

• Excellent communication and interpersonal skills.

• CPR certification.

• Ability to work independently and manage time effectively.

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice

• Provide patient and family teaching according to plan outlined by the RN

• Monitor patient's vital signs and make specific measurements and observations

• Monitor and record intake and output. Report significant changes immediately

• Observe, assess and record patient's physical and mental condition and reports any changes

• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly

• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

• Licensed Practical Nurse / LPN license in good standing

• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred

• Active CPR certification

• Familiarity with electronic health records

• Strong problem solving and critical thinking skills

• Reliable transportation, valid driver's license and current automobile insurance
