Position Summary;

The Primary Care Provider is responsible for the medical services. Horizon Health and Wellness is an Integrated Health Care company. Horizon Health and Wellness provides a multi-directional integrated behavioral health, primary care and wellness services designed to improve individual health outcomes and reduce healthcare costs. This position will actively engage in the provision of culturally responsive, integrate services as well as support and/or participate in initiatives designed to foster and promote integrated service delivery.

Responsibilities:

  • Practice within the scope of his/her license.
  • Initiate entry health care system.
  • Elicit a health, illness, psychosocial, and family history.
  • Perform physical exams and order appropriate diagnostic tests.
  • Record the history, physical examination, diagnostic tests, and an evaluation of health status based on assessment data.
  • Establish and record a plan of care.
  • Regulate and adjust medications and treatments.
  • Prescribe Suboxone according to organizations written protocol. (if applicable)
  • Monitor and record progress, evaluate plan of care and make modifications accordingly.
  • Consult and collaborate with members of the health care team in the assessment of management of mental health problems.
  • Provide teaching and counseling aimed at prevention of illness and modification of risk factors to health, and maintenance of high level wellness.
  • Provide primary care and arrange for continuing definitive care.
  • Assume responsibility for the identification and therapeutic management of illnesses.
  • Mentor students as appropriate.

Requirements

  • Licensed in the State of Arizona as MD, DO, PA or NP and National Certification as a Medical Provider.
  • Possess current DEA registration.
  • Must be at least 21 years of age or older.
  • Have no pending actions by board of medicine in Arizona or any other state.
  • Have current unencumbered license to practice as a physician assistant and to prescribe medications in the State of Arizona.
  • Possess valid drivers license with acceptable driving record, telephone and reliable transportation.
  • Willingness to participate in trainings and continuing education.
  • Ability to analyze and assess situations and patients.
  • Ability to make sound judgments relating to patient care.
  • Ability to complete accurate patient records documentation.
  • Certification in first aid and CPR.
  • Must have good oral and written communication skills.
  • Demonstrated knowledge of referral resources available to participants.
  • Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.
  • Must obtain and maintain a fingerprint clearance card and pass a background check.
  • Perform other duties as assigned.

The Customer Project Associate assists or leads in the coordination and oversight of service-level and routine, less complex customer projects. Coordinate activity among numerous functional areas to meet critical objectives under pressing timelines. Assists in developing and monitoring comprehensive up-to-date work plans for all project work segments, including schedule milestones, expenditures as well as cash flow models. Preferred Special Skills, Knowledge or Qualifications: Experience with customer interface, ability to adapt/respond to a variety of stakeholders, manage complaints, negotiation skills, and problem solving. Business, planning, and project management skills. Computer skills: Windows, Word Excel, CC&B, GIS, and work order management programs such as Maximo and Primavera. Proficiency in the application of PC skills and programs for data management and project scheduling as well as the analytical ability to evaluate the interdependent relationship of project variables. May require Emergency response support for unusual or emergency situations, such as summer storms and/or other catastrophic events. Major Accountabilities 1) Perform as project representative for routine design projects (level 1 and 2 projects which typically include service level and larger scale requests including upgrades, relocations, conversions, maintenance, street lights, dusk to dawn lights, and new services). Plan, schedule, and coordinate all facets of the project from initiation through construction, energization and close out. 2) Perform planning, designing, scheduling, and coordinating all facets of the project from initiation through construction, energization, and close-out. 3) Identify the best source of APS power, the most cost-effective design, the usability of existing equipment based on load analysis and voltage drop calculations, and the adherence to required specifications. 4) Effectively interface with customers/developers and internal departments to assess their needs, determine the scope of the project, obtain plans/maps/drawings, and make decisions regarding the design and installation of APS facilities. 5) Determine the need to obtain easements from property owners and permits from municipal authorities; process through APS right of way to obtain required easement and permit documents; and design the project. 6) Prepare work authorizations (WA) and job estimates. Determine and communicate customer charges required in advance of construction in accordance with APS' extension policy. Prepare and present invoices to customers and manage scheduling to ensure money collected prior to releasing jobs to crews. 7) Conduct inspections during installation of customer electric service facilities to ensure compliance with APS and safety code requirements. Provide instruction to customers/contractors to ensure hassle-free service. 8) Update related project management and customer information databases and close out the project upon completion.
