We are an innovative network of 24 neighborhood primary care centers in the Midwest, focused on providing high quality care to older adults. We are not concierge medicine: we offer this high level of service to our patients at no additional cost to them; it's covered by Medicare regardless of their ability to pay. We want physicians and clinical staff who are intrinsically motivated to provide this kind of excellent care to older adults. We start simple: build meaningful relationships with patients and their families, then layer on resources to help our teams take excellent care.
In this position you’ll be working primarily with geriatric patients, so knowledge of Medicare B is a must. You’ll be working with a full team, including registered nurse support, a medical assistant, a clinical informatics specialist, care manager support, and a nurse practitioner. You’ll also have a flexible schedule to support a healthy work/life balance.
Required skills & experience (the “must haves” to be considered)
1. MD or DO graduate
2. Board certification in Internal Medicine, Family Medicine or Geriatric Medicine is required
3. Active, non-probationary Illinois medical license
4. Must be Spanish speaking

About the Role: Responsible for the planning, organizing, delivery and evaluation of the Primary Care Behavioral Health program. This position will oversee the behavioral health staff that provide therapy and consultation in the primary care Health Centers. Responsible for developing philosophy and strategies involved in the delivery of services. Location: 2400 Belvidere Rd, Waukegan, IL Hours: 40 hours a week, Monday and Tuesday-Friday (8:30am-5:00pm) and Tuesday (11:30am-8:pm) FLSA STATUS: Exempt Skills and Experience * Provides clinical and administrative supervision and development of program staff, including completion of evaluations. * Directs daily operation of assigned program including provision of staff, equipment and supplies to achieve patient visit, revenue and expense goals. * Assures ongoing compliance with Lake County Health Department and Community Health Center, Joint Commission Accreditation of Health Organization, federal and state standards and regulations. * Monitors and insures physical clinic maintenance and safety compliance. * Provides ongoing program evaluation including participation in Continuous Quality Improvement activities and monitoring of program/grant goals and objectives. * Obtains monthly service statistics and provides other statistical information. * Promotes and facilitates the professional development of program staff by means of in-service training sessions. * Interviews job applicants for staff positions and makes recommendations for hiring to Associate Director/Director. * Provides direction on the scope, types, and evaluation of clinical and administrative programming that is offered utilizing published best practices and empirically proven clinical methodologies in delivery of client care. * Demonstrates leadership abilities in conducting administrative and clinical meetings and ensures effective communication among all staff regarding administrative and clinical issues. * Participates in councils and workgroups. * Maintains accountability standards required by LCHD/CHC and funding sources relative to quality and quantity of services provided by staff members. Education, Qualifications, and Certification: * A Master's Degree or Doctorate in Psychology or Social Work or related field. * Licensed as a LCSW or LCPC or Clinical Psychologist or Psychiatric APN or Psychiatrist. * Minimum four years relevant experience, including two years of supervisory experience.
