Qualifications That Make a Difference

• Board-certified in Internal Medicine, Family Medicine, or Geriatrics, with 3+ years of primary care experience, especially caring for the elderly with complex chronic conditions
• Adept at clinical assessment, diagnosis, care planning, implementation, documentation, and evaluation
• Exceptional communicator and collaborative team player, focused on achieving clinical, financial, and patient satisfaction goals
• Supportive of value-based care models and experienced in coordinated practice settings
• Willing to travel to patient locations and participate in on-call rotations
• Current state licensure, DEA license, BLS certification, and compliance with driving standards
