Internal Medicine and Family Practice Physicians are encouraged to consider an available Primary Care Physician position with Liberty Healthcare at the Delaware Veterans Home in Milford, DE.

This limited role will involve providing occasional onsite and on-call services, while serving as a back-up to the Medical Director of the Delaware Veterans Home.

Specific responsibilities will include:

  • Working on an as-needed basis

  • Providing onsite medical care to residents of the home a few hours each scheduled workday

  • Providing offsite telephone consultation to Nurse Practitioners for remaining hours each scheduled workday

  • Assisting with the implementation of resident care plans

  • Maintaining current, accurate and timely documentation in residents' electronic medical records

  • Communicating as needed with local hospitals and other facilities to ensure continuity of care, communication of medical treatment plans, and progression of condition when individuals are hospitalized or seen on a consultant basis

Delaware Veterans Home is a modern state-of-the-art facility which provides high quality long-term care services to approximately 150 Delaware veterans. Medical services at the Delaware Veterans Home are provided by Liberty Healthcare.

The ideal candidate will possess the following:

  • Active Delaware medical license in good standing

  • Board Certification in Internal Medicine, Family Medicine or Emergency Medicine

  • Experience providing primary care medical services to adults and geriatric patients
