THE POSITION: The County of Ventura Ambulatory Care is currently recruiting registered nurses for our Enhanced Care Management (ECM) Team.

ECM provides field-based case management services to members with highly complex medical, mental health, and substance use needs. ECM is a benefit through Gold Coast Health Plan that provides extra services to help members get the care needed to stay healthy. ECM team provides systematic coordination of care to ensure successful participation in medical treatment.

Coordination includes communicating with various doctors and other providers involved in member's medical care. ECM offers care management services by providing a whole person care approach to address social drivers of health.

This position will work a 9/80 schedule and will require travel throughout the County of Ventura to complete field-based assignments. Candidates with experience working with substance use, and/or the mentally ill are encouraged to apply.
