As a member of our Ambulatory Care Enhanced Care Management Team, the Primary Care Clinic Licensed Vocational Nurse I/II plays an integral role in providing skilled nursing care to patients with complex needs, offering compassionate case management and support to help them achieve their health goals. In this role, incumbents will facilitate care planning, goal setting, and motivational interviews, guiding patients through medical, behavioral health, and social services. Additionally, the Primary Care Clinic Licensed Vocational Nurse I/II will conduct field-based care, coordinate with providers, and efficiently document case notes and services provided in the electronic health record system.

IDEAL CANDIDATE: The ideal candidate will have an understanding of Enhanced Care Management (ECM) services and their importance in assisting patients with complex, intricate, and challenging health conditions. Additionally, compassion and a demonstrated history of efficiently overseeing and monitoring vital health data, scheduling various medical appointments, arranging transportation, and maintaining thorough documentation are essential qualities sought in the ideal candidate.

WHAT WE OFFER:

The County of Ventura offers an attractive compensation and benefits package, subject to eligibility requirements, that includes but not limited to:

  • New Hire Incentive: New hires may be eligible for a one-time New Hire Incentive of up to $1,500.00.

  • Referral Incentive: May be eligible for an Employee Referral Incentive of $500.

  • Educational Incentive: Some positions may be eligible for educational incentive. This incentive may be 2.5%, 3.5%, or 5% for incumbents in eligible positions based on completion of an Associate's, Bachelor's, or Master's degree.

  • Bilingual Incentive: Incumbents may be eligible for bilingual incentive depending upon operational need and certification of skill $0.69 per hour (Level I), $1.00 per hour (Level II), or $1.32 per hour (Level III).

  • Certification Pay:$0.813 per hour based on scheduled work week hours for each qualified certification up to a maximum of five (5) certifications.

  • Continuous County Service Incentive Pay: 1% for 7 years, 1.25% for 12 years and 1.5% for 17 years

  • Flexible Credit Allowance: Biweekly contribution towards medical, dental, and/or vision insurance from authorized plans.

  • Pension Plan: Participation in the County's defined benefit plan.

PAYROLL TITLES AND APPROXIMATE SALARIES:

Primary Care Clinic Licensed Vocational Nurse I: $31.08 - $31.80 hourly

Primary Care Clinic Licensed Vocational Nurse II: $37.98 - $39.75 hourly

Primary Care Clinic Licensed Vocational Nurse I/II are represented by the California Nurses' Association (CNA) and are eligible for overtime compensation. Salary placement will be determined according to the current CNA memorandum of agreement.

The eligible list established from this recruitment will be used to fill current and future (including Temporary and Fixed-Term), Intermittent, and Extra-Help vacancies for this position only. There are currently two (2) Regular vacancies.

NOTE: If appointed at the lower level, incumbent may be promoted to the higher level without further examination upon meeting the minimum requirements, demonstrating satisfactory performance, and in accordance with the business needs of the department.

TENTATIVE SCHEDULE

OPENING DATE: April 5, 2024

CLOSING DATE: Continuous and may close at any time; therefore, the schedule for the remainder of the process will depend upon when we receive a sufficient number of qualified applications to meet business needs.

Examples Of Duties

Duties may include but are not limited to the following:

  • Provides field-based nursing care;

  • Provides case management;

  • Coordinates primary care, behavioral health services, and community-based long term services and support;

  • Operates county-owned vehicles to carry out field work assignments and related tasks that require traveling to different locations within the county;

  • Actively participates in assigned patient care team duties and activities, including pre-visit planning, identifying gaps in care, health maintenance needs, and team huddles;

  • Performs basic assessments (data collection) and documents variances;

  • Safeguards patient's health and safety by reporting abuse and neglect, maintaining patient confidentiality, and maintaining current skill and knowledge of safe and competent patient care;

  • May perform Clinical Laboratory Improvement Amendments (CLIA)-waivered testing after obtaining and maintaining annual competency;

  • Performs intake, including vitals and patient measurements, such as height weight, blood pressure, temperature, head circumference, body length, pulse, and oxygen saturation;

  • Administers immunizations and verifies immunizations and dosage for medical assistants administering vaccines per physician or advanced practice provider written or standing order;

  • Ensures appropriate documentation in the electronic medical record;

  • Coordinates patient care by following up on orders, including referrals to ancillary and specialty services;

  • Maintains clean and safe patient areas;

  • Demonstrates adherence to and observes all safety policies and procedures, inclusive of infection control rules and regulations;

  • Answers telephones and takes messages, reviews portal messages and routes as appropriate, and completes assigned tasks to ensure patient needs are addressed;

  • Attends meetings and trainings as required;

  • Communicates with diverse patients regarding disease prevention, health maintenance, community resources, and physician instructions;

  • Provides continuing education for health self-management;

  • Administers first aid or cardiopulmonary resuscitation in an emergency;

  • Reports patient safety incidents to supervisor and via the patient safety reporting software;

  • May participate with registered nurse in developing nursing care plans, maintenance of patients' records, and patient education duties;

  • Performs duties within scope of vocational nursing practice in the State of California;

  • Performs other related duties as assigned;

In addition to the duties listed above, the Primary Care Clinic Licensed Vocational Nurse II also:

  • Provides clinical support to assigned clinical work area;

  • May assist in interviewing new clinical support staff;

  • Supports quality improvement efforts;

  • Properly handles vaccine and medication ordering and storage;

  • Assists with training on workflows and duties; and

  • Performs other related duties as assigned.

Typical Qualifications

These are entrance requirements to the examination process and assure neither continuance in the process nor placement on an eligible list.

EDUCATION, TRAINING, and EXPERIENCE:

Primary Care Clinic Licensed Vocational Nurse I

  • Requires six (6) months of full-time licensed vocational nurse experience in a health care setting.

Note: Primary Care Clinic Licensed Vocational Nurse I incumbents are expected to progress toward Primary Care Clinic Licensed Vocational II by obtaining one (1) year of full-time licensed vocational nursing experience in a health care setting and completing the established competencies set by the Ambulatory Care Nursing Administration for the Primary Care Clinic Licensed Vocational Nurse I.
