Summary Evaluates, plans and carries out programs of purposeful activity designed to assist in the rehabilitation of medically necessary patients as prescribed by the physician. Bases selection of activities from physician#s prescription, history, evaluation findings, experience and interests of the patient; plans and directs special diversional or educational events for patients.

Supervises Certified Occupational Therapy Assistants (COTA) in carrying out an OT program or selected parts of it. Education Must be a graduate of a school of Occupational Therapy holding a BS, MS, or DOT (Doctorate). Licensure/Certification Must be licensed or eligible for licensure in the state of Indiana as an occupational therapist. If eligible for licensure, must obtain license within 3 months of employment.

Must be certified in CPR with American Heart Association; and maintains certificate, according to expiration date. This can be accomplished after employment. If assigned to drive company vehicle, a valid driver#s license is required.

If assigned to drive personal vehicle, proof of insurance is required in addition to a valid driver#s license. Experience Work experience preferred, but not required, other than clinical experience within educational training program. Other Qualifications Must be able to lift 50 pounds; 100 pounds shared lifting. Must have good verbal and written communication skills.
