Sea Mar's network of services includes more than 90 medical, dental, and behavioral health clinics and a wide variety of nutritional, social, and educational services. We are recruiting for the following position(s):
Full-time position available for the Maternity Support Services Program in Burien. This position requires flexibility, initiative and ability to manage time, resources and client caseload.
The individual in this position works closely with new parents of infants ages (two months to one year old) that have identified risk factors. Provides services to minimize the impacts associated with risk factors and to increase the familys self-sufficiency and empowerment. Must have ability to recognize, support and describe healthy parenting from a family strengths perspective.
Must recognize the parent as the childs first teacher and convey the importance of early interaction as the foundation of later learning. Must have knowledge regarding post-partum depression, social isolation and mental health concerns. Must have understanding and respect for cultural differences as well as diverse kinds of families.
This position involves assisting client with accessing resources such as housing, food, clothing, childcare, transportation, education, employment, medical providers, social services, interpreter services and other related health issues with the goal of maximizing the new childs health and well-being.
Qualifications and/or

Requirements:
AA/AS/BA in Social/Human Services and two years of health and/or social services experience working with the childbearing family
Under the direction of an LICSW, follows up on clients in their homes and carries out basic teachings to ensure healthy development of family and infant, utilization of medical care and provide referrals to community resources
Must be bilingual in Spanish/English
Must have access to an insured vehicle and be able to make home visits

VIEW JOBS 11/29/2018 12:00:00 AM 2019-02-27T00:00 Overview The CHART (CHronic-Utilizer Alternative Response Team) Case Management program provides a model for housing support services to extremely low-income, literally homeless and at imminent risk of homelessness households selected from the CHART list by the city of Everett. The primary goal of the CHART case manager is to focus on the priority needs of the identified CHART individuals and work toward those goals that reduce contacts with criminal justice and emergency response systems. The primary intervention is facilitating necessary services that support an individual to reside indoors. The program will use a Low-Barrier and Housing First approach focused on client centered goals but will also work with landlords at scattered-site locations to integrate individuals into housing and to remediate any damage. Incumbent must be able to visit clients who are incarcerated, which may include passing any background screening. Incumbent will track data and report outcomes related to housing status and psycho-social well-being as measured by agreed upon assessment tool and time-frames. Position is Full Time, 37.5 hours per week. $17.25 - $18.11/hr. DOE, plus excellent holiday/vacation/ sick pay and health benefits! For internal candidates/union positions only: Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience. Responsibilities MAJOR DUTIES AND RESPONSIBILITIES: * Act as an essential member of a multi-disciplinary team providing on-going case management services with a harm-reduction focus to adults with severe and persistent mental illnesses and drug and alcohol-related issues. * Provide outreach and engagement services to assigned clients, to initially engage clients and maintain working relationships. * Complete appropriate assessments, develop goal planners with client participation; facilitate linkages to collaborative resources when appropriate. * Assume primary responsibility for coordinating all aspects of consumers' support and goal planners. This includes: providing and/or coordinating the delivery of psychiatric treatment; substance abuse treatment; and performing acute case management to facilitate consumer access to food, clothing, housing, medical care, financial assistance and vocational training or employment. Work with other service providers to provide robust support and a plan for off-hour needs of the client. * Advocate for clients' access to community resources and services, ensuring that clients' needs are met and rights maintained, consult and collaborate with community providers to ensure continuity of care. * Ability to transport clients by providing or arranging transportation to necessary appointments that meet client goals. * Participate in the planning, organizing and facilitating of weekly CHART meetings presenting updates on all active clients. * Travel locally to outreach clients; meeting them on the street/in the community, make home visits, and assist with home inspections. * Provide, as necessary, crisis intervention and risk assessment as well as tolerate unusual or unpleasant behaviors. * Document services provided within required time-frames as outlined by funding contract, program and/or agency expectations. * Coordinate with internal leadership and team to ensure clear and accurate communication relevant to case information and schedules. * Plan, structure and support community based activities such as obtaining and setting up housing, grocery shopping, nutrition assistance, house cleaning and other household activities, money management, social activities and work-related support such as job coaching. * Coordinate with outside service providers including inpatient and outpatient treatment services, state vocational rehabilitation services, social security, veterans benefits, legal and advocacy services and personal primary care medical and dental providers. * Provide practical help and support, advocacy, coordination, side-by-side individualized support, problem-solving, direct assistance, training, and supervision to help participants obtain the necessities of daily living including medical and dental health care, legal and advocacy services, financial support such as entitlements or housing subsidies, money management services (e.g., payee ships), and transportation. * Coordinate with the YWCA, who administers the Home Based Rental Assistance vouchers for CHART individuals. * Other duties as assigned.
