Position Summary:
In alignment with the Associate Director of Adult Care management and Clinical Protocols, this position will participate in the development of population health initiatives with a lens on data coordination; defining project scope, goals, and deliverables that support business goals in collaboration with senior management and stakeholders. They will lead the operational components of AHP's data coordination projects from initiation to completion; ensure implementation fidelity by monitoring progress, tracking performance, troubleshooting and recommending changes to ensure results meet requirements and expectations regarding quality, cost, and delivery. They are responsible for the coordination and operational implementation of data coordination components, and execution, of multiple, simultaneous projects ensuring consistency with organizational strategy, commitments, and goals.
Works to achieve operational function and/or business targets within team(s) with direct impact on departmental results (e.g., QM grid, patient experience, value-based contracts)
Works closely with AHP's, Associate Director of Adult Care Management and Clinical Protocols, Director of Clinical Operations and Quality Improvement, Executive Medical Director, and the Data Coordination Team to execute AHP's identified priorities and clinical and population health initiatives; Partners with care management, AHP's quality and clinical initiative department, and other departments as necessary.
Participates in moderate to large projects across the network; leads operational components specific to data coordination (e.g., pediatric immunizations, cervical cancer screening initiative, breast cancer screening)
Identifies projects within the larger initiative and develops work plans outlining goals, key stakeholders, and a reasonable plan to accomplish the project(s). Takes ownership of projects, keeps them moving forward, ensures projects are completed as committed to, and follows up with project participants.
Directly supports operational efforts for payer contracts, provider compensation plan, quality, and other population management initiatives that require clinical quality metric data collection (e.g., data coordination work).
Collaborates with internal and external, interprofessional practice teams to guide quality and process improvement initiatives, with collection and reporting of data trends related to data coordination efforts directly linked to quality initiatives, value-based contracts.
Oversees the Measurement and Analysis of KPIs for Data Coordination
Prioritizing population health initiatives based on quality measure performance/value-based contracts; collect and report on data trends in quality metric performance for assigned practices supported by Data Coordination within AHP's Network.
Assists in developing key performance indicators, pertaining to AHPs Data Coordination program, for clinical initiatives. Monitors relative KPI's and reporting to identify variation in performance. Ensures consistent evaluation of KPI's and reporting leads to feedback and continuous improvement for the data coordination program.
Collaborates in the analytical, quality and operational projects; participates in monthly meetings, or more often as needed, for communication of activities that impact the data coordinator role, practice workflows and operations.
Participate in the analysis and dissemination of key performance indicators and measures pertaining to each initiative/practice to inform ongoing quality improvement processes and refinement of the data coordination program.
Facilitating and Monitoring Integration of Data Coordination interventions into AHP's Population Health Initiatives
Utilizes patient databases and reporting applications to collect and analyze data for reporting and decision-making and to build queries as necessary for quality and other population health initiatives (e.g., Health Catalyst)
Develops and maintains database queries and spreadsheets to provide trending data as indicators of performance, including but not limited to quality metrics, patients due for care, risk assessment and value-based care initiatives; maintain patient registries to track outcomes and the impact of data coordination efforts
Develops and maintains reports related to clinical quality metrics and population health management including practice and provider performance dashboards from system databases (e.g., pre-visit planning reports, cumulative gap reports)
Maintains knowledge of individual, external practice operations and procedures with the ability to effectively participate in recommending and planning process improvements; acts as a liaison between AHP and community-based practice teams
Develop, disseminate, and maintain educational resources, recommendations and training for internal and external teams
Other Responsibilities

Routinely communicates progress to leadership and directly addresses barriers to success
Collaborates with Associate Director of Adult Care Management and Clinical Protocols to lead project meetings using effective meeting techniques for objectives, agendas, participants, and meeting minutes.

Pay Range
Pay Range: $26.35 - $36.90 Hourly
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
