The Population Health Solutions Analyst, will assist in the development of Population Health Program solutions for new market opportunities and work as a liaison among stakeholders, to include IS, vendors, and internal AmeriHealth Caritas departments to design population health solutions, create proposal responses and implement programs in new and existing markets.

Other responsibilities include :

  • Participates in market analyses specific to Population Health issues, trends and developments nationally and with a focus on the needs of potential growth markets.

  • Develops and presents market-specific reports for internal strategizing. Reports will incorporate the ACFC NGMOC and the market specific information to make recommendations including but not limited to Pop Health programs, value added benefits and in lieu of services for that market.

  • Collaborates closely with Pop Health leadership as well as Market Development.

  • Serve as an SME for the ACFC Population Health program.

  • Obtains comparative information by using competitor and market resources as well as intelligence gathered from attending key conferences and stakeholder meetings.

  • Convenes and facilitates meetings focused on development and refinement of population health program design and implementation.

  • Develops polished presentations, summaries, and reports for internal and external stakeholders.

  • Researches, designs, and develops materials for AmeriHealth Caritas products in collaboration with Corporate Communications.


  • 3-5 years of previous business analysis or system analysis experience in a health care environment.

  • Bachelor's Degree or equivalent experience required.

  • Experience defining problems, collecting data, establishing facts, and recommending solutions.

  • Demonstrated ability to clearly communicate (written and oral) problems and solutions to all levels of an organization.

  • Skilled interpersonal communication and customer service.

  • Ability to work with and through others to achieve results.

  • Travel required, both locally and out of state, up to 25%.

VIEW JOBS 10/31/2019 12:00:00 AM 2020-01-29T00:00 The Health Information Coordinator, in collaboration with management, will develop, revise and educate staff on processes for medical records data. This individual will routinely communicate with the vendor for the NewCourtland LIFE electronic medical record (EMR), and members of the Information Technology department to ensure optimal operation and cohesion of data flow. Along this, the Health Information Coordinator will generate reports, analyze data for compliance with regulatory agencies, and maintain medical records for the program and participants. Job Qualifications In conjunction with the Interdisciplinary Team identify and create appropriate workflows for data into the EMR and continually review processes for potential improvements. ? Without prompting, analyze workflow processes in a clinical setting. Identify, correct, and when necessary create new process for areas of redundancy, delays, gaps, manual and any other inefficient process. ? Establish and maintain both electronic and paper medical records for each participant, including scanning outside consultations and studies that are not entered into the EMR. ? Maintain all documents in EMR within established regulation for LIFE programs and implement new requirements as needed to ensure compliance. ? Troubleshoot and define user issues, utilize the vendor provided JIRA reports, and vendor customer services to resolve problems, where possible. ? Identify and resolve EMR technical errors with the appropriate vendor and in conjunction with the IT Department. ? Develop lesson plans and instructional guides using appropriate instructional methods and approaches, given staff and organizational needs for upgrades, new hires, refresher trainings, and any changes from the vendor. ? Evaluate training needs based on staff roles and processes, errors, upgrades and frequently asked question; modify and implement trainings based on evaluations. ? Document all changes to the EMR and create necessary workflows to incorporate changes. ? Monitor system performance and report anomalies, errors, inaccuracies or inefficiencies. Make recommendations as needed. ? Participate in quality assurance and performance improvement (QAPI) activities. ? Assist with the development of new policies and procedures for EMR usage. ? Review and test all upgraded programming in the Pre-production EMR to ensure quality of changes and accuracy. ? Develop auditing tools and provide appropriate training when changes are made to the EMR ? Request records from primary providers for all new referrals, and as instructed, pull necessary items for scanning into EMR. Follow up on earlier unsuccessful attempts to obtain old records, so that each participant's file is as comprehensive as possible. ? Ensures compliance with HIPAA rules regulation including having participants or responsible parties sign HIPAA documents for all privacy concerns. ? Demonstrate excellent organizational skills and able to prioritize. ? Note when participants have been hospitalized or placed in skilled nursing facility care and obtain copies of inpatient records for review and archiving, and possibly scanning. ? Attend multidisciplinary care team conferences and daily staff meetings, taking cues from meetings regarding follow up on studies, consults or inpatient records. ? Follow up with outside diagnostic groups (radiology, etc) so that copies of reports are filed/scanned into medical records. ? Complete necessary trainings within established timeframes for position (e.g. ICD -10); utilize knowledge gained to train staff and ensure compliance. LI1044 - Page 2 of 5 General Responsibilities ?
