Located in Columbus, Ohio Description: American Health Network has taken the next step forward in healthcare in Population Health Management.# As part of our Population Health initiative, we are identifying high-risk patients and managing their care much more closely than ever before.# This includes assisting and building relationships with patients, meeting them during physician visits, educating patients on managing chronic diseases, and working with their physician to ensure their needs are being met.# The ideal candidate will possess excellent interpersonal skills, have the ability to provide flexible and creative problem solving techniques to alleviate barriers to care, and demonstrate the ability to work effectively as a member of an interdisciplinary team within AHN#s primary care office settings in the Columbus, Ohio area. Ability to work at home at times.


Duties:


Conducting comprehensive assessment of healthcare, educational, and psychosocial needs of the patient and their family. Coach patients and caregivers in self-management Enlisting a team approach by working collaboratively with the family, primary care provider and other members of the healthcare team to ensure coordination of services at multiple provider sites. Facilitate access to community services. Requirements:
Registered Nurse with an unrestricted and active Ohio license. BSN preferred, but not required. 2 years of clinical experience with preference for geriatric experience. 3-5 years Case Management or Disease Management experience CPR Certified CCM (Certified Case Manager) certification preferred Must be computer literate in EHR/EMR and Microsoft Office products Must have an unrestricted driver#s license. Knowledge of state laws and regulations. Experience with Medicare, Home Health, and/or Geriatrics a plus

Direct involvement in continuous quality improvement, regulatory compliance, safety plan management, financial management and daily operations is required.The Population Health CIN/ACO Pharmacy Coordinator position will develop, implement, and update drug utilization review/evaluation programs to optimize the appropriate use of medication therapy and improve disease management in our OhioHealth Group Clinically Integrated Network (CIN) and Accountable Care Organization (ACO) High Risk populations. Programs will collaborate with a multidisciplinary team of providers, including pharmacy technicians, nurse care managers, social workers, behavior health specialists, and health coaches to provide a comprehensive approach to care. In this role, the pharmacist coordinator will provide clinical guidance to the pharmacy team who in turn educates patients through one-on-one phone based disease management and provides recommendations to providers through electronic medical record communications. The pharmacist coordinator will also: Identify target physicians, establish credibility and utilize both verbal and written communications Oversee operations and patient care provided, including pharmacist recommendations and protocol adherence Conduct trainings/evaluations as needed to ensure pharmacists monitoring patient’s medications for indication, effectiveness, safety and adherence issues and recommending lab tests in an efficient manner Serve as the key clinical support person for program inquiries Participate in clinical and operational process improvement and cost savings opportunities Support clinical initiatives related to NCQA, HEDIS, QRS, CAHPS and STAR measures Adhere to Pharmacy Policies and Procedures Develop and evaluate metrics of success on a monthly and quarterly basis Minimum Qualifications MINIMUM QUALIFICATIONS Education, Certification, Registration, LicenseBachelor's DegreeRPH - Registered PharmacistField of Study: PharmacologyYears of Experience: 5MINIMUM QUALIFICATIONS:EDUCATION AND/OR EXPERIENCEB.S. in Pharmacy; Registered Pharmacist in the state of Ohio; Advanced degree or equivalent experience.
